General: Review of the KARES report on 2/6/2020 revealed an individual listed as a current employee was determined to be eligible for employment on 10/29/18. Interview with the staff member in charge revealed that the aforementioned individual was no longer employed with the child care center. General: Review of the presented employee records revealed two (2) staff members with hire dates 7/10/19 and 10/28/19 whose file did not contain a current statement from a health professional stating that they were free of active tuberculosis. Upon request, the staff member in charge did not present a current statement for the aforementioned staff members at the time of the survey.
General: Interviews revealed that a one-year-old child fell from the top of a wooden step stool onto the hard floor causing an immediate bruise on her forehead between 10:00am and 11:00am. Interviews revealed that as the day progressed, the injury began to raise and turn purple. Interview and review of documentation revealed that no first aid was administered to the child at any time after the incident occurred and her mother was not contacted to seek medical care until 4:30pm after the child began to vomit.
General: Interviews revealed that a one-year-old child fell from the top of a wooden step stool onto the hard floor causing an immediate bruise on her forehead between 10:00am and 11:00am. Interviews and review of documentation revealed that the parent of the child was not immediately contacted after the incident even though the child was visibly injured. Interviews revealed that after naptime, staff members noticed that the wound had raised and turned purple color and the parent was not contacted to seek medical care. Interviews revealed that the aforementioned child’s parent was not contacted until five and one-half (5 ½) hours after the initial incident occurred.
General: A review of attendance records on 01/15/2020, revealed children's attendance records for the week had been verified and signed by the parent prior to the children being signed out for the day.
General: Based on a review of documentation, a child was admitted to the center with a temperature, failing to follow center policy. A review of documentation revealed a staff member had been informed by the parent that the child had been running a fever. A review of Loving Hands Center policy states: "Symptoms of illness include but are not limited to: complaining of pain or discomfort, fever, vomiting, uncontrollable cough, sudden onset of rash. A child showing signs of an illness or condition that may be communicable shall not be admitted to the regular child care program".
General: Based on interview and review of documentation, the health, safety and comfort of a child was compromised on 12/18/19, by allowing a child to enter into the program when information from the parent revealed the child had experienced a temperature during the night, and "was running a slight fever early Wednesday morning". Interviews revealed the child's temperature was not taken upon arrival to determine if the child was experiencing a fever at the time of being dropped off. Upon arrival at the hospital the child's temperature was noted as 104.1. Interview also revealed that information obtained from the hospital physician revealed the child should have been transported by EMS due to the temperature and shaking of the child. In a separate incident occurring on 12/16/19, the health, safety and comfort of a child was compromised when staff members failed to be informed that a child could have a seizure while sleeping. Interviews with the staff failed to reveal what to do in case of a medical emergency for a child with an ankle alarm. Interivews also revealed that the child was not enrolled with an ankle alarm and records failed to be up-dated by the center to address medical procedures. A review of the child's medical records failed to provide pertinent medical emergency information. Documentation received by the Division of Regulated Child Care indicated the child care center did not submit the required documentation within the required timeframe of one (1) week regarding a change in director. The vent was not secured to the ceiling located inside the bathroom in the Einstein Room. The ceiling tile was hanging from the frame located above the first window in the Little Explorers Room. There was a water stain in the ceiling tile located above the window in the Little Dreamers Room. The ceiling tile located above the white cabinets in the Little Dreamers Room was cracked across the entire tile.
