General: Observation of the outdoor play area revealed that bare dirt was visible at the bottom of all five (5) yellow slides and beneath the monkey bars, indicating that the protective surface covering in those areas was insufficient. General: Upon request, a current copy of negative tuberculin (TB) test results or a current statement from a health professional verifying that a staff person hired on 08/13/98 was free from active TB was not presented for review. The presented TB test was dated 02/26/21 and, therefore, expired at the time of the survey.

The presented fire drill form revealed the child care center did not have written documentation of the fire drills conducted for September 2016 and December 2016. Interview with the director revealed the fire drills had been conducted; however, she was unable to locate the documentation for the date, time and children who participated in the drills. General: Based on Observation, the child care center failed to maintain adequate supervision in accordance with regulations. Observation at 4:15pm revealed a ten-year-old child left the cafeteria which was supervised by the director and was immediately met in the hallway by the assistant director. However, continued observation revealed the assistant director immediately returned into the room that she was supervising, leaving the ten-year-old child to walk alone thru the school hallways out of the scope of vision and range of voice of all staff members for approximately forty-five (45) seconds until she arrived at her classroom which was supervised by a staff member. General: Based on Interview and Review of Documentation, the child care center failed to comply with regulatory programming requirements. Interviews revealed a staff member made the following comments in the presence of children in care: (1) “If my mother was here, she would wash your mouth out with soap”; (2) “I just ought to whoop you”; (3) “Back in the day, children were spanked”; and (4) “Shit!” Interviews revealed the staff member did not intend for these comments to be threatening to the children in care; however, further interviews and review of documentation revealed these comments were deemed by some parents as profane, threatening, frightening, or abusive language to their children who were in the care of this staff member. General: Based on Review of Documentation, and interview, the child care center failed to maintain the children's files in accordance with regulatory requirements. Based on a review of a child's file with an enrollment date of 07/31/15, there was no physician phone number listed to enable a person in charge to contact. Based on an interview with the staff in charge, she also reviewed additional forms for the required information, but stated none of the forms which were available, contained a physician's phone number.
General: Based on Review of Documentation, the child care center failed to comply with employee documentation requirements. Review of documentation revealed that staff with hire date 8-23-99 had documentation from a health professional that the individual was free from active TB that was not current after 2-12-15. Additional reveiw of documentation revealed that staff with hire date 8-10-10 had documentation from a health professional that the individual was free from active TB that was not current after 6-26-14.

The presented earthquake and tornado drill documentation form revealed the child care center did not have written documentation of earthquake and tornado drills conducted for the fourth quarter of 2016 and first quarter of 2017. Interview with the director revealed the earthquake and tornado drills had been conducted; however, she was unable to locate the documentation for the date, time and children who participated in the drills.
