General: Based on Review of Documentation, one (1) substitute staff had lived in MA, GA, and SC within the last five years. The facility sent off for the FBI records check on 9/2/14 and on 1/7/15 and had no results of the out-of-state background check. This staff was hired on 8/25/14.
