General: Based on review of documentation, the following was found during inspection: 1) A staff (DOH: 08/15/94) was due to have their TB skin test renewed by 04/10/25, but did not have a renewed test until 07/28/25. 2) A staff (DOH: 06/01/21) was due to have their TB skin test renewed by 06/08/25, but did not have a renewed test until 07/24/25.
General: Based on review of documentation, there was no documentation presented at the time of inspection for lockdown or shelter in place drills being conducted at the facility.
