General: Based on observation, interview and review of documentation, upon the surveyor's arrival into the Three-Year-Old Room on 05/02/25, there were twenty (20) children present, and the youngest child was eleven (11) months-old. There were two (2) staff members present. The staff in charge indicated the regular staff member left to find the soccer shots individual, and a floater staff member came in the room to help. The float staff member brought an infant into the room because she was in a class upstairs that would have been over ratio if she had left the baby in that room. The float staff member with the baby was in the room for approximately six (6) minutes until the regular staff member came back into the room. The floater staff member then took the baby back to the Infant Room and ensured that the ratio in the Infant Room was covered. The Three-Year-old Room was out of ratio and group size from approximately 9:27 am to 9:33 am with twenty (20) children present ranging in age from eleven (11) months-old to three (3) years-old.

General: Based on observation, interview and review of documentation, the following was found: 1) A staff member (DOH: 06/05/23) previously lived in Indiana within the last five (5) years from 2018-2020. This information was on the KARES employment authorization from in the staff member’s file. The facility did not have written evidence of conducting a criminal records check, CAN check, and a sex offender check from Indiana. This staff member was not working alone with the children during the renewal survey. 2) A staff member (DOH: 08/07/23) previously lived in Colorado within the last five (5) years from 2015-2020. The facility had a paper in the staff member’s file that was presented as evidence of conducting a Colorado background check. The sheet did not contain a date or the state that the background was conducted from. A handwritten note at the bottom of the paper indicated that they did not print the whole page from the Colorado Bureau of Investigation. The staff in charge was asked if she had additional information for this background check and she was unable to locate any additional information. A CAN check and sex offender check shall be conducted from Colorado. This staff member was not working alone with the children during the renewal survey.
