General: Based on interview and review of documentation, on 07/24/25 at 8:45am a one-year-old child broke the 2 main bones in his arm after falling into a toddler couch. Based on video footage there is a teacher standing at a changing table changing a child's diaper which was facing where the child was playing in a center. The child runs just a few feet, trips and starts to fall so he puts his arm out to possibly catch himself but he lands into the toddler couch and his straigtened arm hits the couch causing his arm to buckle. The child stands up visibly upset with his arm hanging down to his side, he walks towards the teacher at the changing table, she looks up at him and the video ends. The 2 teachers in the classroom did not see the incident occur and did not visibly see anything wrong with the childs arm. Both staff stated that the child was fussy throughout the day and thought it was due to a diaper rash. There are 2 more videos of that day showing the child using the injured arm to walk up a set up steps in the classroom and the second video showed the child handing something to a teacher using the injured arm. The child was not picked up from the center until 4:17pm per ProCare. The parents later called the center asking if something happened that day because the child was not using that arm. The parents took the child to seek medical attention and an x-ray determined his 2 main bones in his arm were broken. One (1) staff member reported that they have witnessed one (1) staff member caring for up to eight (8) infants at one time. 2.) It was found that a staff member working in the Green Frogs Classroom was unable to reach another staff member to be relieved for a bathroom break. The staff member found that the staff member in the adjoining classroom, the Monkey’s Classroom needed to use the restroom as well. The staff members took turns using the restroom while the other oversaw both classrooms from the adjoining doorway. The incident occurred sometime between 9:35 and 9:50 a.m. The staff person did not call the parent but sent a picture of the child's injury through the Kid's report application; however, the parent did not receive the picture or report due to issues with the application. The parent became aware of the injury when she picked up the child at 4:56 p.m. and was given an incident report. On 3/2/20, it was reported to the DRCC office that a preschool child enrolled at the facility, pulled on a lamp cord which caused the lamp to fall on the child. The child received a small cut above his right eye that required stitches. The staff person stated during interview that the lamp was positioned on top of a shelving unit and the child was laying on a cot beside the shelf at the time. The majority of the electrical cord had been secured to the wall with tape; however, a small portion of the cord, that included the on/off switch, was hanging loose behind the shelf. The staff person did not observe the child playing with the cord; however, heard the child cry out when the lamp landed on his head. The surveyor toured the classroom during the visit and and found that the lamp had been removed from the room; however, facility staff presented the lamp during the visit. General: Based on interview and review of documentation, it was found that regulatory violations did occur. It was determined that the center did not adhere to their head lice/nit policy when an outbreak occurred. It was found that the facility changed the policy in the middle of the outbreak in order to remain open and stay in compliance with ratios at the facility. Although the policy initially indicated that staff would be sent home when head lice was identified, this policy was not followed.

General: Based on Review of Documentation, a two-year-old child was left unsupervised in a classroom for twenty-four (24) minutes. According to video footage provided by the facility, nine (9) children and one (1) staff person can be seen in the Busy Bees classroom at 3:34 p.m on 8/28/18. The children are standing in a line in front of the cubbies waiting to exit the classroom to go to the playground. At 3:35 p.m. the staff person walks to the door of the classroom to lead the children outside and one (1) of the children is seen leaving the line and going to the carpeted area in the back of the classroom. The staff person and the other eight (8) children leave the classroom at 3:36 p.m. and the one (1) child is left in the classroom unsupervised. The child stays in the carpeted area of the classroom from 3:36 p.m. until he walks to the front of the classroom at 3:48 p.m. where he is no longer in camera range. General: Based on Review of Documentation and interview, this regulatory requirement was not met. On 3/27/18 an administrative staff person reported an incident that occurred at the facility involving a fourteen (14) month old child. The incident report stated that the staff person stood the child on the diaper changing table after getting a diaper change when the child lost her balance and hit her cheek on the floor. The child's parent was contacted and she was taken to her pediatrician to be examined. Even though the child was not diagnosed with an injury other than a red mark and bruise on her face, the staff person did not use a safe procedure to pull the child's pants up by allowing her to stand on top of the diaper changing table. The staff person received a written reprimand on the day of the incident (3/27/18), a second written reprimand and suspension on 3/28/18 for continuing to use this same un-safe procedure and her employment was terminated a few days later. General: Based on Review of Documentation, there was one (1) staff person with a hire date of 4/27/17 whose Child Abuse and Neglect check indicated the staff person's current address is in Ohio. However, there was no documentation to verify that the application for an out of state background check for the state of Ohio been submitted and completed. An interview with the staff person in charge indicated that this staff person has not been left alone with children. There was one (1) staff person with a hire date of 6/16/17 whose application for the results of a criminal records check was not submitted until 6/23/17. There was an additional satff person with a hire date of 5/1/17 whose application for the results of a criminal records check had not been submitted until 5/2/17. The application for the results of a criminal background check for both of these staff were not submitted until after their hire date and not prior to their hire date.
