During a tour of the facility, the surveyor observed a small group of children standing in line in the hallway on the main floor that were waiting to use the bathroom. Approximately six ceiling tiles were missing in the hallway and four trash cans had been positioned in the hallway to catch water that was dripping from the ceiling. The staff person in charge instructed the staff to take the children to the bathrooms on the upper level of the building due to the water leak.
