General: Based on interview, on 07/24/25 at 8:45am a one-year-old child broke the 2 main bones in his arm after falling into a toddler couch. There were 2 teachers in the classroom and both stated that they did not see the incident occur and did not see anything visibly wrong with the child's arm. Interview with the parent stated that a Facebook message was sent to her by a teacher around 12:30pm stating that the child had been cyring most of the day and could be due to a diaper rash but not for sure. The child was not picked up until 4:17pm that day and once home, the parents noticed the child did not want to use his arm. Parent called the center to see if anything had happened that day but no one knew of any incident. The child was taken to Urgent Care where an x-ray determined he had broken the 2 main bones in his arm. Employee Records In Compliance
General: Based on interview and review of documentation, on 07/24/25 at 8:45am a one-year-old child broke the 2 main bones in his arm after falling into a toddler couch. Based on video footage there is a teacher standing at a changing table changing a child's diaper which was facing where the child was playing in a center. The child runs just a few feet, trips and starts to fall so he puts his arm out to possibly catch himself but he lands into the toddler couch and his straigtened arm hits the couch causing his arm to buckle. The child stands up visibly upset with his arm hanging down to his side, he walks towards the teacher at the changing table, she looks up at him and the video ends. The 2 teachers in the classroom did not see the incident occur and did not visibly see anything wrong with the childs arm. Both staff stated that the child was fussy throughout the day and thought it was due to a diaper rash. There are 2 more videos of that day showing the child using the injured arm to walk up a set up steps in the classroom and the second video showed the child handing something to a teacher using the injured arm. The child was not picked up from the center until 4:17pm per ProCare. The parents later called the center asking if something happened that day because the child was not using that arm. The parents took the child to seek medical attention and an x-ray determined his 2 main bones in his arm were broken. General: Based on observation, the following was found: 1) A staff (DOH: 05/06/24) had a cleared background check in KARES, but the eligibility date was 07/05/24 which was after her hire date. 2) A staff (DOH: 06/25/24) had a cleared background check in KARES, but the eligibility date was 07/02/24 which was after her hire date.
General: Based on observation, there were five (5) cribs in the Infant Classroom that had loose fitting sheets on the day of inspection which poses a safety hazard for the infants.
General: Based on observation, there was a light that was not working on the day of inspection in the Gross Motor Room.
General: Based on review of documentation, the following was found: 1) There was documentation of drills on file at the facility; however, the time the drills were conducted was not documented. 2) There was no evidence presented at the time of survey of earthquake, tornado, and lockdown drills being conducted for the first quarter (Jan-Mar) or a fire drill for July at the time of inspection.
General: Based on interview, it was reported that a staff member will tell children in the childcare center that they are “bad”. Additionally, it was reported that in June of 2025 the staff member used inappropriate physical discipline with a child in care. The staff member was working in the Green Frog Classroom when a child was standing on the coach. The staff member approached the child and "patted" him on his bottom twice, while appearing agitated.
General: Based on interview, this regulatory requirement was not met. The following was found: 1.) It was found that the Sea Stars Classroom will exceed five (5) children approximately once week until a staff member is able to report to the classroom to assist, which may take up to five (5) minutes. One (1) staff member reported that they have witnessed one (1) staff member caring for up to eight (8) infants at one time. 2.) It was found that a staff member working in the Green Frogs Classroom was unable to reach another staff member to be relieved for a bathroom break. The staff member found that the staff member in the adjoining classroom, the Monkey’s Classroom needed to use the restroom as well. The staff members took turns using the restroom while the other oversaw both classrooms from the adjoining doorway. It was reported that the staff members oversaw approximately twelve (12) or thirteen (13) students, aged one (1) to three (3) years-old. Each staff member reported that they believed that they were in the restroom for one (1) to two (2) minutes each. General Administration In Compliance
General: Based on interview, it was reported that a former staff member working in the Green Frog Classroom did not realize that children had separated from her group of one (1) year-old children on several occasions. On some of these instances children would have been left in the hallway, within line-of-sight of another staff member. It was reported that on one occasion in April of 2025, a one (1) year-old child was found alone in the hallway by a parent and was taken to the Sea Stars Classroom. It was unknown how long this child had been left unsupervised in the hallway. On another occasion, two (2) one-year-old (1) children were found running up the stairs and hiding in the hallway unsupervised. The children were found by a staff member working in the Sea Stars Classroom. General: Based on observation, the child care center failed to maintain state regulations. At the time of survey, the children in care in the Explorer Classroom were in the midst of rest time. The surveyor observed the cots of the children in care to be very close together, approximately five (5) inches apart, which would not allow for free and safe movement between.

General: Based on observation, the surveyor found grass protruding through the mulch and overgrown bushes in the corner of the preschool playground. This causes a visual barrier for staff to track children. The staff person stated that she tries to keep the children away from the area. General: Based on interview and review of documentation, this regulatory requirement was not met. During the investigation it was found on the facility video footage that a staff member was sitting on the floor between two children’s sleeping mats during the nap time. The teacher turned to a child on her left side and was talking to that child. The other child had leaned over the staff member’s right leg (calf area) and proceeded to bite her. The staff member then lifted the child off her leg with her left hand and swiftly smacked the child on the back with her right hand.

Based on interview, this regulatory requirement was not met. The staff person in charge described what she observed on facility video footage. On 9/8/21, at approximately 10:32 a.m., one staff person was changing a child's diaper while a second staff person was in the back-half of the Green Frogs Classroom (one-year-old room) with a small group of children. During this time, three children were involved in an altercation which resulted in one of the children getting bit on the arm and leg. The staff person, positioned in the back-half of the classroom, stated during interview that she was not aware of the altercation between the children. The staff person changing a child's diaper stated during an interview that she "heard a commotion"; however, she did not know what had occurred because she could not see the children from the diaper changing table. The activity that the staff were involved in did not allow them the ability to adequately supervise the children’s activities throughout the classroom. Based on interview and record review, this regulatory requirement was not met. The following was found: 1. An interview with staff during the investigation revealed that on 9/8/21 at approximately 10:30 a.m., a staff person reached to pick up a child and scratched another child on the face by accident. The staff person completed an accident report stating, "In attempt of stopping the child from hurting another student, I accidentally scratched her cheek when I tried to catch her". 2. A child who is not able to stand without support was placed in a non-supported standing position after a diaper change. The child fell forward onto the tile flooring and sustained an injury to their nose that resulted in medical assessment at an Urgent Treatment Center. General: Based on interview and review of documentation, on 12/9/2020, a nine-month-old male child received an injury after hitting his nose on the metal part of the bottom of a door. The incident occurred sometime between 9:35 and 9:50 a.m. The staff person did not call the parent but sent a picture of the child's injury through the Kid's report application; however, the parent did not receive the picture or report due to issues with the application. The parent became aware of the injury when she picked up the child at 4:56 p.m. and was given an incident report. The parent reported when she picked up her child, his nose was swollen and bruised and both of his eyes were black. It should also be noted that there was no first aid administered to the child. Based on observation and documentation, a caterpillar climber/tunnel was not placed on a protective surface on the deck used by the toddlers for gross motor play. The climber/tunnel is approximately three feet tall and was placed directly on the wood decking.

On 3/2/20, it was reported to the DRCC office that a preschool child enrolled at the facility, pulled on a lamp cord, which caused the lamp to fall on the child. The child received a small cut above his right eye that required stitches. An interview with staff during the investigation revealed that the staff person working in the classroom at the time of the incident was sitting on the floor patting a child's back because it was nap time. The surveyor toured the classroom during the visit and and found that the staff person was not positioned in an area to allow her to adequately supervise the child that received an injury because two shelving units were positioned between her and the child. General: Based on review of documentation, observation and interview, this regulatory requirement was not met. During the inspection, the surveyor observed an adult male working alone with a group of nine children in the indoor gross motor room. The staff person in charge identified the adult male as being employed by a company called "Happy Feet," and stated that he comes to the facility one day a week to teach a soccer class to the children whose parents paid for the service. General: Based on interview and review of documentation, it was found that regulatory violations did occur. It was determined that the center did not adhere to their head lice/nit policy when an outbreak occurred. It was found that the facility changed the policy in the middle of the outbreak in order to remain open and stay in compliance with ratios at the facility. Although the policy initially indicated that staff would be sent home when head lice was identified, this policy was not followed.

General: Based on Review of Documentation, a two-year-old child was left unsupervised in a classroom for twenty-four (24) minutes. According to video footage provided by the facility, nine (9) children and one (1) staff person can be seen in the Busy Bees classroom at 3:34 p.m on 8/28/18. The children are standing in a line in front of the cubbies waiting to exit the classroom to go to the playground. At 3:35 p.m. the staff person walks to the door of the classroom to lead the children outside and one (1) of the children is seen leaving the line and going to the carpeted area in the back of the classroom. The staff person and the other eight (8) children leave the classroom at 3:36 p.m. and the one (1) child is left in the classroom unsupervised. The child stays in the carpeted area of the classroom from 3:36 p.m. until he walks to the front of the classroom at 3:48 p.m. where he is no longer in camera range. General: Based on Review of Documentation and interview, this regulatory requirement was not met. On 3/27/18 an administrative staff person reported an incident that occurred at the facility involving a fourteen (14) month old child. The incident report stated that the staff person stood the child on the diaper changing table after getting a diaper change when the child lost her balance and hit her cheek on the floor. The child's parent was contacted and she was taken to her pediatrician to be examined. Even though the child was not diagnosed with an injury other than a red mark and bruise on her face, the staff person did not use a safe procedure to pull the child's pants up by allowing her to stand on top of the diaper changing table. The staff person received a written reprimand on the day of the incident (3/27/18), a second written reprimand and suspension on 3/28/18 for continuing to use this same un-safe procedure and her employment was terminated a few days later. General: Based on Observation, during a tour of the Funky Monkeys room the wall by the plastic kitchen set had an area down by the baseboard where the paint had peeled off leaving the drywall exposed. In the same room on the wall under the window where a step stool is located there was an area where the paint had chipped exposing the drywall. General: Based on Observation, during a tour of the Funky Monkeys room there were several areas of tape residue with an accumulation of dirt on the back of two (2) double doors located in the classroom.
General: Based on Review of Documentation, there was one (1) staff person with a hire date of 4/27/17 whose Child Abuse and Neglect check indicated the staff person's current address is in Ohio. However, there was no documentation to verify that the application for an out of state background check for the state of Ohio been submitted and completed. An interview with the staff person in charge indicated that this staff person has not been left alone with children. There was one (1) staff person with a hire date of 6/16/17 whose application for the results of a criminal records check was not submitted until 6/23/17. There was an additional satff person with a hire date of 5/1/17 whose application for the results of a criminal records check had not been submitted until 5/2/17. The application for the results of a criminal background check for both of these staff were not submitted until after their hire date and not prior to their hire date.

General: Based on observation and interview, the following was found: 1.) It was confirmed that the door leading to the playground from the Sea Stars Classroom does not lock properly. 2.) It was confirmed that the door leading to the lower parking lot from the downstairs hallway does not shut properly and that children have been able to elope through the exit.

General: Based on observation and interview, the child care center failed to maintain state regulations. During the survey of the class restroom in the Green Frog's Room, multiple purses belonging to staff were found hanging on a hook, accessible to children as they entered and exited the restroom. It should be noted that the classroom teachers acknowledged that the purses were theirs when questioned by the surveyor.
General: Based on observation, the child care center failed to maintain state regulations. The seat on the toilet in the Happy Camper's Room was extremely loose. During the inspection of the bathroom, the surveyor noted the seat was off-center and pushed all the way to the right side of the toilet. General: Based on review of documentation, observation and interview, this regulatory requirement was not met. On 3/2/20, it was reported to the DRCC office that a preschool child enrolled at the facility, pulled on a lamp cord which caused the lamp to fall on the child. The staff person stated during interview that the lamp was positioned on top of a shelving unit and the child was laying on a cot beside the shelf at the time. The majority of the electrical cord had been secured to the wall with tape; however, a small portion of the cord, that included the on/off switch, was hanging loose behind the shelf. The staff person did not observe the child playing with the cord; however, heard the child cry out when the lamp landed on his head. The surveyor toured the classroom during the visit and and found that the lamp had been removed from the room; however, facility staff presented the lamp during the visit. General: Based on Interview, it was found that when sanitizing toys that are considered mouth contact surfaces by a child not yet toilet trained staff scrub the toys with a brush in warm soapy water and rinse in clean water. Staff then stated that a sanitizing solution is sprayed on the toys and they are left to air dry. When sanitizing toys the toys must submerged in the sanitizing solution.
