General: Based on interview and review of documentation, it was found that three staff members did not complete the required 15 hours of training for the July 1, 2020 to June 30, 2021 year. The staff person in charge reported that she did not think she was going to bring these staff members back, but did end up re-hiring them. It is important to notes that these staff members have worked in childcare within the last five years. The original documented hire dates for these staff are 06/04/2019, 03/11/2019, and 12/11/2018. General: Based on Observation, the facility did not have the statement of deficiency and plan of correction posted from the relicensure survey conducted on 3/16/17. The facility did not have the staff-to-child ratios posted that indicated the ages of the children. The posted ratios were as follows: State 8:1, Natural Start Preschool 12:1, and Natural Start Extended Care 12:1. General: Based on Observation, two (2) high school students were observed working in the preschool classroom during the relicensure survey. The staff in charge indicated that the students were doing their school observations and they visit the facility Monday through Thursday for about thrity (30) minutes each day. The staff in charge indicated that these students have done this since October 2017. The students were observed interacting with the children and helping the children with activites. The student volunteers did not have CAN checks on file.
General: Based on Review of Documentation, it was found that the permission forms signed by the parents for the facility staff to apply sunscreen to the children did not list the name of the sunscreen or when it should be applied. The sunscreen is provided by the parents.

General: Based on Observation, on 5/11/16 a visit was made to the facility at approximately 10:36 a.m. but there was no one at the facility. The surveyor called the number listed on the sign out front. The surveyor spoke with the staff person in charge who indicated the facility had closed for the day because they were only going to have one (1) child in attendance. It was also found through interview that the facility is operating from 8:00 a.m. to 4:00 p.m. The time listed on the application and in Division of Regulated Child Care computer system has the facility's hours of operation as 7:00 a.m.-6:00 p.m. Monday through Friday. The facility did not notify the Division of Regulated Child Care the facility would be closed for the day nor were they notified of a change in hours.
General: Based on Observation, an unlocked first aid kit was located on a shelf above the toilet in a bathroom used by the children. Inside the unlocked kit was a tube of first aid cream, a tube of Johnsons burn cream, a tube of Polysporin Bactricin Zinc, packets of extra strength non-asprin medication and packets of Motrin, Tylenol, Immodimum A.D. and cooling burn relief gel. it was found that the main classroom has a large column in the middle of it. The column is approximately four (4) feet square. During the survey, children were observed to be playing and participating in activities on both sides of the column. With there being only one (1) staff person in the room, all of the children were not properly supervised. The director stated that she was aware of the issue.
