General: Based on observation and interview during a tour of the facility, the following was found accessible to the children in care: 1) In Room 9 (Kindergarten B), an adult's purse was found on the bottom shelf of an unlocked cabinet under the sink. 2) In Room 11, an adult's bag containing personal belongings was found in a tall, gray cabinet. The bag was on a shelf located in the middle portion of the cabinet. There was a lock on the door but the lock was not engaged. 3) On the two-year-old side of room 6, an adult purse and an open bag containing an adult pair of scissors and other personal items had been placed on a low, open shelf located against the wall. A clear plastic cup that contained a dark liquid and a straw was also observed on the same shelf. The staff person in charge was with the surveyor when this was observed. She immediately asked the staff person in the room to move the items to a location away from the children. 4) On the three-year-old side of room 6, an adult's purse, backpack and a spray bottle labeled bleach water was observed on low shelves inside of a gray cabinet. The cabinet was unlocked at the time. The staff person in charge stated that the cabinet is normally locked and should be at all times. General: Based on observation and interview, at the time of the survey on 9/10/2020, the following was found: 1) During a tour of Room 9 (Kindergarten A & B), six children aged six-years-old were not wearing a mask according to 922 KAR 2:405E, Section 8(4) which indicates that only children ages five and under shall not wear a mask. 2) Machine-washable toys (Barbie clothes and felt pieces) were found during a tour of rooms 7 and 11. 922 KAR 2:405E, Section 6(3) states, “Machine washable toys shall not be used.” 3) Classrooms 6, 7, and 9 were observed to be divided into two spaces each, with each divider being made of black plastic secured around the sides of frames made of black metal. In rooms 7 and 9, children on one side of the room would have to walk through the other side to enter and exit. In room 6, a diaper-changing table was observed on the side of the room with two-year-old children, positioned against the end of the divider closest to the door. It is important to note that the restroom and sink used for handwashing after diapering, for meals, and other uses, is located on the other side of the divider with three-year-old children. In each room, traffic patterns will cause children to come into contact with children in a different group. 922 KAR 2:405E, Section 4(5)(e) states, “…A temporary wall: (e) Shall not create a traffic pattern that would cause noncompliance with health and safety requirements during a medical state of emergency. This same staff person also indicated that if a child does not lay quietly and stay on their cot she will make them stay in a "Safe Place" for three (3) minutes the next time they go outside.
