General: Based on Observation, the water temperature at the sinks used by the children in Preschool 1 and Preschool 2 was measured with a calibrated thermometer and found to be 124 degrees Fahrenheit and 132 degrees Fahrenheit, which exceeds the maximum allowable temperature of 120 degrees.

General: Based on Review of Documentation, and interview, the facility failed to meet this regulatory requirement. The staff person in charge stated that lunch is catered from a company called Phoenix Catering. A review of the food service permit the facility had on file for Phoenix Catering found that it expired 12/31/12; therefore, the documentation on file was no longer current.
