General: Based on interviews and a review of documentation, it was determined that a two-year-old child placed her middle fingertip in a classroom restroom door hinge when a staff member inside the restroom closed the door on 10/30/25, at 9:00 a.m. The child’s fingertip was injured, and she was taken to Cincinnati Children’s Hospital, where she received one (1) stitch and bandaging. This incident was not reported to the Division of Regulated Child Care.
