General: Based on Observation and interview, the facility failed to obtain approval before implementing an addition to the square footage at a chid care center's premises. During the visit the surveyor observed a group of preschool children and staff using a room in the upper level of the building that the Fire Marshal and Cabinet had not approved. The staff person in charge identified the room as the "Blue Pod" and stated that they started taking groups of children into this room approximately two (2) times a week last year to do a group fitness class called "CATCH".

General: Based on Observation, it was found that the facility uses parent volunteers as "lunch helpers." The director stated that these individuals help the children if they need help with their lunches and clean up the lunchroom after each group. The volunteers sign up to serve as "lunch helper" as many times as they like during the school year. During the survey, the lunch helpers were observed to redirect children, help them with their lunches and hug the children. One of the volunteers was reported to have been a "lunch helper" on more than one (1) occassion. These volunteers did not have criminal records checks or central registry checks on file. The director stated that the facility did not submitt for these checks to be completed.
