The Office of Child Care Licensing (OCCL) received a complaint on November 14, 2025, regarding concerns involving safe sleep practices in the infant classroom, improper discipline, failure to provide an injury report, and not following termination policies. An investigation was conducted on November 17, 2025, during which Licensing Specialists (LS) #1 and #2 interviewed Administrator #1, Administrator #2, and Staff #1, obtained documentation, and reviewed video footage. Based on the information obtained, there is no evidence to substantiate the allegations of improper discipline and lack of an incident report from said improper discipline. LS #1 and LS #2 reviewed the footage of this incident, and Administrator #1 and Administrator #2 explained there was not a scratch on the child s chest. No forceful or inappropriate physical contact was viewed between Staff #2 and Child #2. This complaint is substantiated for not following safe sleep practices, as Staff #1 told LS #1, and Administrator #1 confirmed safe sleep checks were not being completed or documented during infant nap times. Additionally, Staff #1 noted children some children need a full five minutes to transition to a crib after falling asleep in other equipment, rather than transferring them immediately. This complaint is substantiated for a lack of behavioral supports for Child #2, as Administrator #1 stated documentation had been collected on Child #2 s behaviors, but no supports or plans for supports were in place, and noted staff were overwhelmed with the behavior and did not know how to address them. There is no evidence to substantiate this complaint for not following termination policies based on review of the letter provided to Parent #1 regarding termination of care for their children, as well as review of the facility s termination policies in their parent handbook. The parent handbook states immediate termination may occur due to bullying, inappropriate behavior, or disrespect towards children and employees, which are concerns noted in the termination letter provided to Parent #1. An additional concern noted during this investigation was Staff #1 was alone in the infant classroom without the required DEEDS Early Learning qualification. An additional concern noted was the length of time infants spent in confining equipment, as an infant was observed in confining equipment for an hour and a half on November 6, 2025. Two additional concerns noted were there was no lesson plan posted in the infant classroom and lighting in two classrooms was insufficient during nap time, as it was too dark to view all individual children.
