Ensure walls and ceilings are in good repair/cleanable/free from stains and discoloration. - Replace the missing ceiling tile in the Open/Close Room - Replace the stained ceiling tiles in the 2/3's classroom. - Spot paint all of the walls where tape has removed the paint revealing the dry wall. - Remove/clean all tape and adhesive residue on the hallway walls. All surfaces must be non-pourous and cleanable.

The following staff needed the following pieces of documentation: -Current CPR/First Aid--Staff #1, 3, 4, 5, 6, 7 -Professional Development Plan--Staff #2, 6 - Job Description--Staff #6 - Release of Employment--Staff #3, 4, 6 - Service Letters/Two Documented Attempts to obtain Service Letters--Staff #3, 4 - OCCL Health and Safety Training for the Early Childhood Professional--Staff #1, 2 **UNDETERMINED AT TIME OF VISIT: - Professional Development Training (with 2 hours being in Health and Safety) for Staff #1, 2 and 6 Send documentation to OCCL
On November 14, 2024, a complaint was called into the Office of Child Care Licensing (OCCL) regarding concerns of a lack of supervision, preventing families access to the facility, rodent and/or insect infestation, as well as children being fed expired food. An investigation was conducted on November 20, 2024. This complaint is substantiated for a rodent infestation because LS #1 and Licensing Supervisor #1 observed rodent feces in every classroom, with significant amounts present in the infant classroom. Administrator #1 acknowledged an ongoing rodent issue and confirmed the facility was in the process of negotiating a contract with a new extermination company. A quote for extermination services was provided as evidence of the facility's efforts to address the problem. There is no evidence to substantiate the allegation of families being denied access to the facility. Administrator #1 explained the procedure in place for entry, where families ring a doorbell and a front office staff member grants access. Administrator #1 also stated most families opt not to enter the building and instead drop children off at the door. There is no evidence to substantiate this complaint for lack of supervision. Multiple staff members, including Staff #1, #2, and #4 were interviewed, and none reported witnessing or observing a time when children were left unsupervised. All staff members consistently described the procedure for requesting assistance if needed when they were the sole staff member in a classroom. Attendance records provided by Administrator #1 confirmed appropriate coverage during lunch breaks and other times requiring additional supervision. There is no evidence to substantiate the concern of children being fed expired food because LS #1 and Licensing Supervisor #1 observed a substantial supply of appropriate and unexpired food stored in the infant classroom and the kitchen. Administrator #1 noted no children are served expired food. Additional concerns no ted at this visit were the overall cleanliness and maintenance of the facility. All classrooms were observed to require deep cleaning to address the accumulated rodent feces as well as general cleanliness. The school-age classroom needs repairs, specifically to the flooring and wall moldings. Several pieces of broken furniture were observed in the hallway, along with an unmaintained fish tank, all of which needed to be removed. Additionally, the Busy Bees and the Dragonfly classrooms have a lack of sufficient supplies and materials for the children to engage with.

Ensure all children's files are complete. Child #1, 2, 7, 12 needed updated Immunization Records with Blood Lead Screening. Submit documentation to OCCL.

Replace the diaper changing mats in the Infant Room and the Lady Bugs (2/3 Year Olds). Send pictures to OCCL.
Designated administrators trained in day to day operations must be present/have training document in their complete staff file. Submit form for the two individuals on-site at the time of visit to OCCL.
Ensure adequate lighting in the 2/3 Year Old Bathroom (Lady Bugs). Send picture as documentation to OCCL. Ensure that the gym doors are secured and closed when authorized staff are not present in the gym. LS Tavares and Coover were able to walk right in upon arrival. Post signs on the inside stating "ALL DOORS MUST REMAINED CLOSED AND LOCKED WHILE GYM IS UNATTENDED" and on the outside "ALL VISITORS MUST REPORT TO THE MAIN ENTRANCE." Send picture documentation to OCCL.
Ensure girls bathroom stall doors are free from hazards. Paint is chipping. Send a picture to OCCL when doors are free from chipped paint. Ensure all cleaning bottles are labeled and not in reach of children in care. This was corrected at the visit in the one-year old room.
Ensure gym is cool and kept below 82. Temperature was well over 82 degrees. Monitor room for temperatures to be below 82 degrees. before use. Recommendation Use library space for gross motor for all children in care until temperatures reach below 82 degrees in the gym. Ensure floor fans are not in use and inaccessible to children in care.
Ensure Child files are complete. Child #1 needs blood lead screening results. Send to OCCL when complete.
Ensure staff files are complete. All staff except staff# 1-4 need to complete OCCL's Health & Safety seven hour course. Send certificate to OCCL when complete. Staff #5 needs to complete CPR & First Aid. Staff #6 needs 1 reference letter. Send to OCCL when complete. Reminders Staff #4 and Staff #7 needs First Aid and CPR by August 15. This will be non-compliant if not received by Aug 15, 2023. Send certificates of completion to OCCL when complete.

A staff member in the infant classroom was working on school-work on a laptop while they were supposed to be providing direct care to the children. This was corrected at the visit.
Staff attendance was inaccurate. A staff member that was seen outside of the center and then in a classroom was never signed out or back into the classroom. LS will check attendance at next enforcement visit.

A diapering procedure and a foot activated trash can is needed in the second bathroom on the left-hand side of the hallway.
The renewal application is due 60 days prior to license expiration. This was corrected on 7-18-22.
Ensure the second set of restrooms on the left side of the hallway has toilet paper and paper towels available at all times.
Ensure all meals meet required components and change the menu if something has changed for the day. This was corrected at the visit.
Ensure there is music equipment available in the three-year-old classroom, and both science and music equipment available in the two's classroom. Send pictures.

Infant daily logs for four infants in care showed no diaper changes for three hours prior to the time of the visit. Send copies of daily logs for the rest of the week.
Crib mattresses must be at the lowest position. Send a picture of this when corrected.

The Office of Child Care Licensing (OCCL) received a complaint on December 8, 2022, regarding concerns about an injury to the face Child #1 with differing explanations. An investigation was conducted on December 13, 2022. This complaint is substantiated for improper notification regarding a serious injury, as defined in the Delacare Regulations. During interviews, it was noted Child #1 was injured prior to nap time, which was noted to end at 2:00 P.M 2:30 P.M. The written accident report stated the accident occurred at 3:15 P.M. The center sent a text message to Parent #1 at 3:48 P.M. Parent #1 arrived at 4:45 P.M. to pick up Child #1. An additional concern noted during this visit was improper staff attendance documentation. More than one interviewee stated Staff #2 was in and out of Child #1 s classroom, and was not present during this incident. Attendance documentation shows Staff #2 present in the classroom all day with the exception of a lunch break.

The Office of Child Care Licensing (OCCL) received a complaint on October 19, 2022, regarding Staff #1 using a cell phone for personal reasons while directly responsible for children, and Staff #1 using inappropriate and threatening in front of the children. An investigation was conducted beginning October 19, 2022, and concluding on October 31, 2022. This complaint is substantiated for Staff #1 using a cell phone for personal reasons while signed in and being responsible for the care of children. Additionally, Staff #1 was recorded using inappropriate language and a threatening tone while working in the classroom.

On March 10, 2022, the Office of Child Care Licensing received a complaint regarding improper care of an infant in care. At the conclusion of an investigation conducted beginning March 14, 2022, this complaint is substantiated with additional concerns. Infant feeding schedules were not available for the infants in care; one was provided but was not completed by the parent. Infants were noted to be given oatmeal, juice, and medication without proper documentation, in addition to two safe sleep violations occurred during the visit. A daily log provided showed an infant not being changed or fed for nearly four hours. Menus viewed at the visit were not dated and did not reflect the meals that were being served to the children. Copies of attendance documentation showed a staff member alone with more than four infants without fingerprinting eligibility or a DEEDS early learning qualification, and an additional staff member was alone with more than four infants. Attendance documentation from a follow-up visit showed an additional staff member alone with infants without fingerprinting eligibility. At this visit, one classroom was over-ratio because one staff was alone with 9 two-year-old children, and one staff, who was not 18, worked alone with infants for two hours. Lastly, one staff member who was present had not yet been fingerprinted despite working at the center for a few months.
