On August 14, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of failing to use appropriate behavior supports and failure to meet meal requirements. Based on observations and documents reviewed, this complaint is substantiated for failure to meet meal requirements. The meal served was missing one required component and the food served did not match the food listed on the menu for the day. Based on interviews conducted, observation, documentation collected, and video footage reviewed, there is no evidence to substantiate this complaint for allegations of failing to use appropriate behavior supports. Concerns noted during the visit were the meal for the two- to three-year-old class was served on Styrofoam plates, and an unqualified staff member was observed alone with seven two-year-olds and three-year-olds.

On March 17, 2022, a complaint was received regarding the allegations of improper staff-to-child ratios and not following the diaper changes requirements. An unannounced visit was conducted on March 21, 2022, to investigate the complaint. The complaint is substantiated because the Preschool classroom was not maintaining the correct staff-to-child ratio upon the arrival of the licensing specialists. It was also determined by reviewing the attendance sheets that the Infant/Toddler classroom had been over ratio on multiple dates when the infant who is enrolled was present in the classroom. The licensing specialists were able to determine by reviewing the Bright Wheel app that on March 18, 2022, the children had only been changed twice. The concerns observed during the complaint investigation were that the administrator left the classroom leaving the preschool-age children alone with an unqualified staff member twice during the visit. It was also observed that the attendance sheets were not being completed correctly or accurately because some staff and children were not signed in. An additional concern was that the center had a staff member present who had not been fingerprinted prior to her start date.
