On February 19, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of supervision, incorrect ratio, room capacity, and lack of security procedures. An investigation began on February 24, 2025, and concluded on March 3, 2025. Based on the documentation reviewed and interviews conducted, this complaint is substantiated for incorrect staff-to-child ratio because preschool two classroom attendance records documented one classroom was over correct ratio for 27 minutes on February 19, 2025. Based on the measurements completed on March 3, 2025, there is no evidence to substantiate this complaint for exceeding room capacity. Based on interviews and observations during the visit, there is no evidence to substantiate this complaint for lack of security procedures to limit access to the children. Administrative staff stated the front door is monitored by staff and the security override is not used except during fire drills. Based on interviews, there is no evidence to substantiate this complaint for lack of supervision as all staff interviewed denied anyone was asked to take a child who was not their own from one classroom to another room.

The Office of Child Care Licensing received two complaints on January 13, 2025, regarding allegations of incorrect staff-to-child ratios, exceeding maximum group size, staff qualifications, and an unsafe physical environment. An investigation was begun on January 15, 2025, and concluded on January 30, 2025, when submitted paperwork was reviewed. Based on the results of the investigation, including staff and administrator interviews and a review of attendance records, this complaint is substantiated for incorrect staff-to-child ratio. Based on observations, interviews, and a review of documentation, there is no evidence to substantiate this complaint for unqualified staff alone with children, exceeding maximum group size, or an unsafe physical environment.

The Office of Child Care Licensing received a complaint on September 12, 2024, regarding allegations of improper staff-to-child ratio. An investigation was conducted on September 19, 2024. Based on the results of this investigation, this complaint is substantiated for improper staff-to child ratio. LS #1 and LS #2 observed two classrooms out of correct staff-to-child ratio at the time of the visit, as well as reviewing two weeks of attendance documentation showing classrooms with incorrect staff-to-child ratio on September 18 and 19, 2024. Additional concerns noted at the visit were LS #1 and LS #2 observed an administrator remove one child from a classroom to correct ratios and take the child to front desk in the lobby which is not licensed child care space. In addition, the specialists observed empty cribs in the infant room that were less than 18 inches apart. Staff in the classroom stated the cribs are not moved further apart when infants are in the cribs.

A licensee shall ensure that all staff use prevention strategies, appropriate redirection rather than restraint, and positive developmentally-appropriate methods of behavior supports for the children, which encourage self-control, self-direction, positive self-esteem, social responsibility, and cooperation. Center acted quickly by placing staff member on leave, 3/8/2024, the day of the incident and terminated staff member on 3/15/2024.

An administration of medication forms is needed for Children #9, 13, 16, and 21. All infants need MAR's when using diaper cream. An infant feeding schedule is needed for Children #9 and 16. Update infant feeding schedules monthly.
Ensure the playground fall zones have plenty of protection. Rake or add mulch as needed.

On April 15, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding children being cared for in inappropriate indoor spaces, insufficient staff to maintain ratios, and failing to notify OCCL of flooding. An investigation was conducted on April 24, 2024. Based on the documentation reviewed, interviews conducted, and observations during the visit, this complaint is substantiated for children being cared for in inappropriate indoor spaces because staff stated children were sometimes being cared for at the front desk area at the end of the day. There is no evidence to substantiate this complaint for insufficient staff to maintain ratios, or failure to notify OCCL of flooding, as attendance records documented correct ratios were maintained and the flooding that occurred did not affect the center s ability to operate. Non-Compliance Cited Reg 49 Indoor Space A licensee shall ensure interior space designed for children's use is available to children when the center is in operation. 1. The space must be arranged to allow each child adequate space for free movement and active play. 2. A center must have at least 35 square feet of usable floor space for each child in each area or room used by children.

Based on staff interviews and documentation received, this complaint is substantiated for incorrect staff-to-child ratio. Four staff members interviewed stated they have had incorrect staff-to-child ratio in their classrooms. Based on documentation, there were at least four instances where there was incorrect staff-to-child ratio in classrooms with durations ranging from six minutes to thirty-nine minutes.

On December 12, 2022, the Office of Child Care Licensing (OCCL) received a complaint regarding sanitation practices, not following health exclusion policies, and the uncleanliness of the toddler classrooms, including furniture and toys. An investigation was conducted on December 13, 2022. This complaint is substantiated for failure to follow sanitation regulations because the specialists observed dirty classroom areas, furniture, and toys. The staff also stated different procedures for cleaning toys and classroom areas, that did not follow the administrator s description of classroom cleaning procedures. There is no evidence to substantiate this complaint for failure to follow health exclusion policies because neither the staff or the Reporter clearly identified situations requiring exclusions from the center.
