On February 4, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding an allegations of insufficient staffing due to one staff member being present with infants and older all day, improper staff-to-child ratios, not following transportation agreements, and not following Department of Labor (DOL) meal break requirements. This complaint is substantiated for not having sufficient staff to fulfill required duties. On February 13, 2025, the center was operating with one staff member supervising nine children when licensing specialists arrived, and the staff member left the classroom unsupervised to answer the door. This complaint is substantiated for not following DOL meal break requirements because attendance records reflected that staff members have not received meal breaks on several days. This complaint is substantiated for not following transportation agreements. Due to insufficient staffing, transportation was unable to be provided for the school-age children to school, requiring the families to pick the children up from the child care center and take them to school. There is no evidence to substantiate this complaint for insufficient staffing or improper staff-to-child ratio with infants present because there are no children under two years old present, and attendance sheets reflect the correct staff-to-child ratio in the preschool classroom.

On April 24, 2024, the Office of Child Care Licensing received a complaint regarding improper discipline, the center and bus being cluttered and not clean, and a staff member smoking on the premises. An investigation was conducted on April 30, 2024. This complaint is substantiated for improper discipline because while the Administrator denies hitting a child, he acknowledged he said a child was acting like a dummy. This complaint is substantiated for being cluttered indoors and outdoors. Piles of papers were observed on the stove in the kitchen, a fluorescent light bulb was on the table in the toddler room, and the bus used to transport children had seats in extreme disrepair with excessive trash on the floor. This complaint is substantiated for smoking on the premises because a used cigarette receptacle and a discarded cigarette butt were observed on the front sidewalk. Additional concerns noted were a diaper changing table without a sink within five feet and an expired bus registration.

The Office of Child Care Licensing received a complaint regarding erratic driving and an expired vehicle registration. This complaint is substantiated for an expired registration because, during the investigation on February 24, 2023, Staff #1 acknowledged it was expired, as did the review of registration documentation. There is no evidence to substantiate the erratic driving allegation because Staff #1 stated she does not drive erratically and there is no evidence to support the claim.
