A complaint was received on October 18, 2021, alleging a violation of COVID-19 requirements, not providing a parent with a handbook, and the center not following their own policies. An investigation was conducted on October 28, 2021. This complaint is substantiated because staff acknowledged a parent was not wearing a mask in the center. Additionally, this specialist observed two staff members without masks. Staff acknowledged they have not had a hard copy of the current parent handbook to give to parents since the start of the school year, but informed parents there was a copy available for review in the office until more printed copies were returned from the printer. There is no evidence to substantiate for the center not following their own policy because Ms. Bigalow stated it is not a rule that parents not enter the classroom, but that parents have been asked not to enter the classrooms and at times certain circumstances may make that more challenging for some parents. An additional concern is that there was not an early childhood teacher qualified staff member that was designated as in charge in the absence of the administrator or curriculum coordinator when this specialist arrived at the facility.

At the conclusion of an investigation conducted on October 4, 2021, this complaint is substantiated because LS Bailey witnessed one staff member wearing a mask improperly, during this visit.

The changing tables in two of the classrooms need to be cleaned. Send a picture when complete. Shoe moulding in the hallway bathroom stall needs to be repaired. Send a picture when complete.
Repair or install wooden boards at the bottom of the fencing on the large playground next to already installed boards.
Children's files will need the following: - Child #2, 5, 16 need lead screening. - Child #19 and 20 need updated health appraisals.
