On December 30, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns about improper diapering procedures and not providing sufficient meals and snacks in the two-year-old classroom. An investigation was conducted on January 8, 2025. There is no evidence to substantiate this complaint for either allegation. All staff interviewed stated children are changed in the morning, before lunch, and after nap time. Depending on when children are picked up, staff stated there will be a fourth diaper change. Administrator #1 stated snack is served at 2:30 pm every day. If a child did not eat, it is because they chose not to eat the food and staff do not force children to eat. The Pro-Care documentation confirmed Child #1 was changed in the morning, before lunch, after lunch, and before pick-up on December 20, 2024, and all of the meals and snacks were offered to Child #1.

On May 22, 2024, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on May 22, 2024, and accepted for investigation regarding improper discipline, lack of supervision, and failing to notify a parent of a serious injury immediately. An investigation was conducted on May 30, 2024. There is no evidence to substantiate this complaint for lack of supervision and improper discipline because Staff #4 witnessed the incident occur and it was due to Child #1 tripping and hitting his face on a table. All staff interviewed stated they had never seen or participated in improper discipline. This complaint is unsubstantiated for failing to notify a parent of a serious injury immediately because according to the injury reports collected at the visit, Parent #1 was notified of Child #1 s injuries on May 20, 2024, at 12:13 pm, and May 13, 2024, at 4:03 pm.

On May 24, 2024, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint and accepted it for investigation on May 24, 2024, regarding concerns of improper discipline. An investigation was conducted on May 30, 2024. Because the facility does not have cameras to record classroom events and because two administrative staff and two teaching staff were interviewed and stated they had not witnessed any concerning behaviors or improver discipline at the center, and Staff #1 stated she has never grabbed, yelled at, or used improper discipline on a child, there is no evidence to substantiate this complaint.

Disposition: No Evidence to Substantiate
A complaint was received on March 14, 2022, regarding the allegations of lack of reporting an injury and lack of supervision. An investigation was initiated when Ms. Keating contacted OCCL on March 14, 2022. All documentation for the investigation was received on March 15, 2022. There is no evidence to substantiate the complaint because five staff members stated Child #1 did not have any marks, bumps, or bruises on his face at the time he left the center on March 11, 2022. Two of these five staff, who are assigned to Child #1 s classroom, also stated they had not observed Child #1 fall or injure himself in any way and that Child #1 had not complained of an injury while in their care.

On November 16, 2021, OCCL received a call regarding a self-reported incident of a child gaining access to medication while in care. On November 23, 2021, OCCL received a complaint regarding the same incident. An investigation began on November 16, 2021, and concluded on December 15, 2021 This complaint is substantiated for having medication from an unknown source accessible to a child that was ingested. A staff member immediately noticed the child had something in his mouth and removed it and followed all the additional steps to protect the health of the child by calling the parent and the Poison Control Center immediately.
