On December 17, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of improper discipline, lack of supervision, and child accident and injury. An investigation was conducted on December 20, 2024. Based on interviews conducted, documents reviewed, and observations on the day of the visit, this complaint is substantiated for lack of supervision because a licensee or staff member shall supervise a child in care at all times, and there was no staff member present in the play space to supervise the children when this injury occurred. There is no evidence to substantiate this complaint for improper discipline and child accident and injury because the provider assisted in removing the child from harm and notified the parents. A concern noted at the visit was Provider #1 did not notify OCCL of an injury that required further medical treatment.
