On May 25, 2022, OCCL received a complaint regarding lack of supervision and improper staff-to-child ratio. The complaint is substantiated for lack of supervision because during the investigation visit staff acknowledged occasionally Child #1 had suddenly left his group to go to Ms. Marrs in the office. There is no evidence for improper staff-to-child ratio because Child #1 did not join another classroom.

On May 31, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding incorrect staff-to-child ratio. An investigation was begun on June 12, 2024, and concluded on June 24, 2024. This complaint is substantiated for incorrect staff-to-child ratio because based on documentation received, there were multiple instances where it appeared there were no staff members in the room with children for up to two hours. Additional concerns noted were staff members failing to keep an accurate record of staff s exact hours worked in each classroom and not keeping accurate child attendance records because according to documentation received, staff members failed to sign out of tracking sheets and staff members failed to sign children out of the classroom at the end of the day.

On January 30, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding unsafe transportation of children. An investigation was conducted on February 5, 2024, and on February 6, 2024. Based on the results of this investigation, this complaint is substantiated because Ms. Monti stated the center is transporting children in vehicles that do not have a traveling first aid kit and a dry chemical fire extinguisher. This complaint is also substantiated for transporting children without having a policy regarding this transportation in the parent/guardian handbook.

On July 5, 2023, and July 6, 2023, two complaints were called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit (IAU). On July 5, 2023, the Office of Child Care Licensing (OCCL) received the complaints and conducted an investigation on July 6, 2023. This complaint is substantiated for inappropriate adult behavior because staff members statements and video footage confirmed Staff #1 lifted her foot to keep Child #1 in the classroom and her foot made physical contact with Child #1. This resulted in Child #1 falling onto the floor. Additionally, Ms. Monti confirmed she took Child #3 out of the center in her personal vehicle without Parent #1 s permission or knowledge.
