Over the counter medications (tylenol, ointments) were observed on an open shelf in the bathroom. This shelf was accessible to children in care. During the visit, this was brought to the provider's attention and they identified a lock box they could use but the key was missing.
Significant wear and tear was observed of the building materials in the Living room/play room. The wooden built-in shelves were observed to have multiple areas of missing paint. The Fireplace mantel was missing large amounts of paint and appeared to have been partially removed/scraped. Paint chips were observed flaking off and some on the floor in the corners of the carpet. The carpet was observed to be pulled away from the corner and had dried paint on it and paint chips. The left side of the hearth was accessible and it was observed to be higher than the surrounding carpet. One side of the room was sectioned off with large furniture. The opening to this area had a child sized chair and adult sized chair blocking children's access. An infant was observed climbing and pushing on the child sized chair. The area on the other side of these items was a designated office area and included several hazards to children, including but not limited to electrical cords, open electrical outlets and choking hazards.

The correct sized mounted extinguisher has an inspection date of October 2023. The inspection tag indictates the inspection was good for one (1) year. The extinguisher needed to be re-inspected by the end of October 2024.
The documentation for emergency evacuation drills conducted between 09/2023 through 12/2024, show drills occurring at different times of the day excluding the timeframe of 12:30 p.m. to 2:00 p.m. It was confirmed with the provider that 12:30 p.m. to 2:00 p.m. is rest time. Provider acknowledged no emergency evacuation drills have been conducted during while children have been sleeping or resting.
The documentation of professional development hours for June 2023-June 2024 available within BFIS and on site, confirm completion of two (2) hours of the required fifteen (15) hours.
The infant child sleeping in the stroller/bassinet was resting directly upon a loose fitting sheet. There was bunched up sections of the sheet near one corner and raised sections of sheet near the child's feet. When the child was out of the stroller, the sheet was able to be moved with a pull by finger and thumb to one side of the sheet. There was no visible means of securing the sheet to the surface.
This site visit inspection recorded no violations or advisories.
