Upon review of the outdoor play area it was observed that the fence has become detached at the top portion of two (2) fence posts at the back of the fence beside the shade hut. Additionally, the shade hut's wall on the inside of the left side of the structure was not attached to the frame.

Upon review of BFIS it was found that one (1) staff member's CPR/First Aid certification had expired. When discussed with the director she stated the staff member has not gotten recertified yet.
A review of staff BFIS credential accounts and staff files, reflected that many staff documents were not yet reflected in accounts. While the director stated the program's process for submitting documents with cover sheets to Northern Lights at CCV, it was noted that the cover sheet utilized was not the most recent version. Further, the program does not appear to have an established system for conducting regular checks of staff BFIS accounts to ensure all submitted documents have been added or following up with the advisors at Northern Lights at CCV when items do not appear in accounts as expected. This is a repeat violation.

A review of the staff handbook reflected a lack of written policies on the topics included in this rule. While the program has a staff orientation check list that lists the topics, the written policies were not made available for review.
Staff E shared that Staff A works half of the day in the infant room and Staff F works the other half of the day in the infant room. Neither Staff A or Staff F have qualification documents in BFIS to show qualifications as Teacher Associate. Staff E stated that only Staff A has college coursework which may qualify Staff A as Teacher Associate. Neither Staff A or Staff F work with the infants for 75% of the program hours of operation.
Staff E confirmed that Staff A, C, D, E, F, and H had not submitted documentation with cover sheets to Northern Lights at CCV (NLCCV) in order to achieve and/or maintain up-to-date BFIS Quality and Credential accounts. As a result, Staff Qualifications and compliance with CPR and first aid certifications, and other required trainings were not able to be assessed.

On 02/11/17, when 15 preschoolers were being brought inside for lunch time, one child was left alone outside for 5 to 10 minutes before staff realized a child was missing from the classroom. Due to the significant amount of time the child was outside alone the level of risk is high. This is a serious violation requiring a parental notification letter.
A review of staff BFIS Quality and Credential Accounts and staff files reflected that Staff A, B, C, and D did not have current certifications in pediatric first aid and infant and child CPR. Staff E confirmed that Staff B had not yet completed CPR and first aid certifications and that Staff A, C, and D had not renewed their CPR and first aid certifications prior to expiry. Staff A, B, C, and D are full time staff members at the program. Staff A, B, and C were all present and counted in ratio during the visit.
