When staff are supervising children, they shall always ensure their care, protection, and guidance. On 11/24/2025, staff did not ensure that care and protection of children when child 1 sustained an injury to the lip. Staff 1 and 3 confirmed the child's injury however were unaware how the injury occurred and failed to follow up with investigation and/or reporting.
Staff 2 was employed for ten days before the facility obtained a completed and signed sworn statement.
