Staff #5 has resided outside of the state of Virginia in the last 5 years. Staff #5 record does not contain the results of an out of state background check for the state that they resided in.
1. The centers "Transition Tracking Policy" states that "Teachers are required to conduct a name-to-face roll call with child count at the following times: when leaving a location or entering a new location". Staff shall "Conduct a verbal head count at the following times: when passing through any additional door/gate on the way to a location. "In addition, the last teacher leaving a location is responsible for conducting a visual sweep, which consists of viewing all areas in the classroom to confirm no child is left behind". 2. On 6/11/2025, staff #1 brought a group of 8 children from the playground into the pre-k classroom so that child #1 (3 years old) could go to the bathroom. Approximately 5 minutes later, staff #1 left the classroom with 7 children to go back onto the playground, not realizing that child #1 remained in the bathroom. Staff #1 did not complete a name-to-face roll call when leaving the pre-k classroom. Staff #1 did not complete a verbal head count of the children when leaving the pre-k classroom. Approximately 3 minutes later, child #1s guardian arrived at the center to pick the child up for the day. It was discovered that child #1 was still in the pre-k bathroom.
An email thread containing childrens and staff names and information regarding a classroom was forwarded from the center administration to a family outside of the center, unintentionally. The email thread was not treated confidentially.

Evidence: There is a whole in the resilient surfacing on the preschool playground, approximately 4" wide.
Evidence: A child under 12 months was sleeping on a cot. The child had been sleeping on a cot for two weeks. (Child 2 DOB: 3/24/23)
Evidence: 1 of 2 child records reviewed was not updated annually. Child #2's record was dated 11/4/22.
Evidence: 7 of 7 injury reports reviewed were missing documentation of any future action to prevent recurrence. The online form in use by the facility does not include this component.
