Staff #3 was alone with 4 children in the Preschool/K Prep classroom, and was not supervised at all times by an individual with all required background checks completed within the last five years.

Evidence: 1. Staff # 1, Staff # 2 and Staff # 3 had a total of 13 children on the playground. 5 of those 12 children were under the age of two years. The ratio for children 16 months to 24 months is 5 children to 1 staff member. 2. At approximately 10:47 am, there were 12 children on the playground with two staff members. 3. Staff # 1 had taken one child to use the bathroom, leaving Staff # 2 and Staff # 3 with 12 children and out of ratio. 4. Staff # 2 stated that Staff # 1 was gone for approximately one minute.
Evidence: 1. Child # 1 was 14 months old. 2. The most recent immunization records in Child # 1's record was from March and May 2022. Additional immunization records were not obtained after that date.
Evidence: 1. Staff # 1's record and timesheets showed that her first day of employment was 2/22/22. 2. Staff # 2 also verbally confirmed that Staff # 1's first day of employment was 2/22/22. 3. Staff # 1's fingerprint results were dated 3/2/22.
Evidence: A white plastic pipe attached to the water table on the playground was broken, posing an injury hazard to the children.
Evidence: 1. Staff # 1's record indicated her first day of employment was 9/12/19; and that she had resided in Maryland within the last five years. 2. The center did not obtain results of the sex offender search for Staff # 1 by the implementation date of 12/31/21. The search was dates 4/15/21.
