Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and to investigate a complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The child care agency did not notify the Department of a serious incident/injury. The educators did not fulfill their duties to ensure a safe environment for children which contributed to a child being injured.

See Below for Detailed Information 1240-04-01-.15 Program, Language, and Literacy Development Observation: Child was not properly assessed for injuries or comforted after being injured on the playground. Video footage shows child crying with no educator providing comfort. Corrective Action: Educators will assess for any injuries and provide love and cuddles until ready to return to play. Educators will document in a notebook in the classroom any injuries or marks noted during diaper changes. 1240-04-01-.09 Incident Reporting Observation: Injury on 5/10/22 was not reported to parent and injury report was not completed until after parent inquired about the injury. Corrective Action: Educators will assess all injuries and report to parents the day of occurrence. 1240-04-01-.11 Supervision Observation: Educators were not following playground supervision policy. Educators were seated away from where children were playing and not positioned to provide adequate playground supervision or interaction. Corrective Action: All educators have completed the Essentials of Outdoor Safety training on Pro-Solutions and will also receive Playground Supervision training with CCR&R on 5/31/22. Educators will walk the playground and do name to face checks while children are on the playground.
