Technical Assistance Provided 3/23/26: 1. LC provided TA to the provider on social emotional activities to implement in the classrooms to help children calm down and shared resources to be implemented. 2. LC provided TA to the provider on reviewing appropriate discipline and behavior management techniques for when children need extra support. 3. LC served the individual safety plan at the provider, and the employee is no longer employed as of 3/20/26. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans.

Summary of Complaint Investigation and Safety Plan Serving Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint Investigation and Serving Safety Plans. LC served the individual safety plan at the provider, and the employee is no longer employed as of 3/20/26. During the visit on March 23rd, 2026, Child Care Licensing Consultant Lexi Casteel was investigating an incident that was self-reported by the day care and reviewed video footage of a staff member yanking toys out of a child's hand and jerking him up while trying to get the child to clean up. The educator aggressively moved the child and used an aggressive tone.

During the visit on December 2nd, 2024, Child Care Licensing Consultant Lexi Casteel determined that the director and assistant director were aware of the staff member fall asleep and compromising supervision. The director and assistant director failed to report the incident to TDHS. The director of the agency is responsible for ensuring all incidents are reported to TDHS on the same day. During the visit on December 2nd, 2024, Child Care Licensing Consultant Lexi Casteel observed video footage while investigating a complaint of a staff member falling asleep while they should have been supervising children. Staff should always have direct sight of children and sound.
