Continued Non-Compliance: On 9/28/22, during an unannounced inspection, Staff #7 file did not contain an updated pediatric first aid/cpr training. The training on file expired 2/2021. On 12/2/22, during an unannounced inspection, Staff #7 has still not completed training in pediatric first aid/cpr.
While investigating a complaint on 11/3/22 it was observed in Twaddler classroom a total of 20 children present. Staff person # 1 was asked for the names of children responsible for. Staff person#1 could not identify children specifically assigned to child care group. It was observed in the pre-school#2 classroom a total of 16 children present. Staff person # 2 was asked for the names of children responsible for. Staff person#2 could not identify children specifically assigned to child care group.
On 9/22/22, Staff 1 reported that at approximately 12:00pm Child #1 was having a "tantrum." Staff #1 reported witnessing Staff #2 picking up the rest mat belonging to Child #1 and "hitting Child #1 in the head with it" while Child #1 was sitting upright. Staff #1 reported that approximately 5-10 minutes later, Child #1 was still upset. Staff #1 reported that Staff #2 got a spray bottle out of the cabinet. Staff #1 witnessed Staff #2 walk past Child #1 and spray Child #1 2-3 times in the face. Staff #1 reported Child #1 became more upset after being sprayed by Staff #2. When interviewed by Staff #3, Staff #2 acknowledged "spritzing" Child #1 with the bottle. Staff #2 stated the spray bottle contained only water.
On 9/28/22, during unannounced inspection, an individual record was not on file for Staff #8.

DURING RENEWAL INSPECTION ON 7/20/2022, CERT REP ONSERVED THAT REST EQUIPMENT WAS NOT LABELED FOR SPECIFIC CHILDREN.

On 3/7/22, staff member #1 self admitted to using harsh tones with children in care.
On 3/7/22, staff member #1 self-admitted and was observed by other staff, moving a chair from underneath a child who was sitting on it, allowing the child to fall on the floor. As well as picking children up by one arm as a form of redirecting children.
IT WAS REPORTED BY ANOTHER TEACHER THAT THE IDENTIFIED STAFF WAS SEEN AND HEARD PUTTING CHILDREN IN THEIR SEATS ROUGHLY, MISHANDLING THEM ON THE PLAYGROUND BY DRAGGING THEM, AND INAPPROPRIATE/BORDERLINE YELLING AT THE CHILDREN.
The bottle of child #10 was not labeled with child's name.
