There was peeling paint observed along the wood trim in the left side classroom, back right-side classroom, and front right classroom. There was peeling paint observed on the wooden rocking chair in the back middle classroom.
Observed a foam green and red climbing pad that was ripped and had exposed foam. This item was accessible to children who are still placing objects in their mouths.
The files for Child #1 and #2, Preschool aged children, contained Health Reports that had been updated in an interval greater than 12 months.
Child #3 had an expired Epi-Pen in the classroom.
The State Police Clearance in the file for Staff person #2 lacked a rap sheet.
The file for Staff #3 contained a foreign degree and their credentials could not be determined.
The door leading outside from the Infant space did not close or seal the whole way leaving a gap creating a pinching hazard to the children in care.
SECOND CITATION: At approximately 3:15 PM Staff #1 identified 7 Preschool Aged Children as part of their assigned primary group; Staff #2 identified 7 Preschool Aged Children as part of their assigned primary group. Cert Rep observed 15 Preschool Aged Children receiving care in the Outdoor PlaySpace therefore leaving a child not identified as being in an assigned primary group. Previously cited 8/20/25
In the Left Side Classroom, a broken plastic ledge along the bottom of the cabinets that created a sharp point was observed, posing a cutting hazard to the children in care.
In the Left Side Classroom, the bottom of the diaper changing pad was observed to have a black/white sticky layer and therefore not in clean repair. in the Right Side 2 the top lid of the rest cots was observed to be cracked on two ends creating a sharp corner for the children in care. In the outdoor Play space, there was a ripped deflated soccer ball and a small blue plastic piece of a toy that had broken off, these items were not in good repair.
A crib, in use by an infant, lacked 2 feet of space on three sides.
The Emergency Contact Form for Child #2 lacked the health insurance coverage for the child.
The bathroom in the Left side Classroom lacked hand washing signs posted at both of the toilets.
Cert Rep observed preschool aged children playing with a chair, scooter, and bubble seat on the second level of outdoor play structure approximately four feet off the ground. These items were not fixed or a manufactured part of the play structure and posed a falling hazard to the children.
Five plants found to be toxic were present in five separate childcare spaces in the facility.
The first aid kit located in the Infant 2 Room was observed hanging on a low hook making it accessible to children.
The rubber-base board located in the Infant Room under the window was detaching from the wall.
The file for Staff #1 contained a Health Assessment at the time of inspection, but it was observed they did not have a Health Assessment before their first day working in childcare. The Health Assessment in the file for Staff #2 lacked a date the assessment was completed. The last two Health Assessments in the file for Staff #3 were dated more than 24 months apart.
The file for Staff #5 lacked proof of qualifications for education in the form of a diploma, transcript, or letter signed by a representative or the experiential, educational or training entity.
The facility lacked written statements provided by the parents giving the formula and feeding schedule for the infants in care.
Upon review of the facility's fire drill log it has been more than 60 days since the last fire drill conducted on 6/30/23.
Upon review of the facility's 30-day fire testing documentation, the following tests were observed to have been conducted more than 30 days apart: 11/4/22 - 12/29/22 (55 days), 12/29/22 - 2/15/22 (48 days), 2/15/23 - 3/30/23 (43 days), 4/3/23 - 5/24/23 (51 days) and 6/30/23.
Observed in the Right side 2 Classroom there was a cabinet that had a broken child lock on it, giving the preschool children access to the items inside which included a pair of sharp teacher scissors.
Although the file for Staff #1 contained complete clearances at time of renewal inspection, it was observed that Staff #1 did not obtain an FBI clearance within the 45 days of the provisional period and continued to work in childcare, and therefore was incorrectly provisionally hired. The file for Staff #3 lacked a Child Abuse clearance and out of State Clearances at the time of hire, and therefore was incorrectly provisionally hired.
Staff person #2 had not completed the mandated reporter training within 90 days of their date of hire (see LIS code sheet).
Observed a can of shaving cream in the Right-side Classroom 2, labeled keep out of the reach of children, in the storage cabinet with the other art/craft supplies. Staff stated it was used for the children's art projects.
In the preschool and pre-k rooms, the wooden lids on the sensory tables had splintering in the corners of the lids.
The first-aid kit in the older infant room lacked soap.
The file for staff person #6 completed current pediatric first-aid and CPR training dated 9-9-21 that was after the May 2021 expiration of the prior training.

The file for Staff #3 contained a Volunteer Child Abuse Clearance. The file for Staff #4 contained a National Sex Offender Registry Certificate that had not been updated in the previous 60 months. The file for Staff #5, who has lived outside of Pennsylvania in the last five years, lacked West Virigina Clearances.
In the Left Side Classroom, a ripped brown child sized chair that had exposed foam was observed. This item was accessible to children who are still placing objects in their mouths.
Based on Smoke Detection testing documentation, there were gaps in the documentation exceeding 30 days where the smoke detectors were not tested, 9/20/24 - 11/15/24 (56 days), 11/15/24 - 1/9/25 (55 days), 1/9/25 - 3/6/25 (56 days), 3/6/25 - 5/3/25 (58 days), and 5/3/25 - 7/21/25 (79 days).
The Right Side 2 Room's First aid kit was hanging on a low hook making the first aid kit accessible to children in care.
At approximately 10:00AM Staff #6 identified 4 Older Toddlers as part of their assigned primary group; Staff #5 identified 5 Older Toddlers as part of their assigned primary group. Cert Rep observed 10 Older Toddler children receiving care in the classroom therefore leaving Child #3 not identified as being in an assigned primary group.
At approximately 10:30AM Staff #7 identified 4 Young Toddlers as part of their assigned primary group, Staff #5 identified 1 Older Toddler and 4 Young Toddlers as part of their assigned primary group, and Staff #6 identified 1 Older Toddler and 2 Young Toddler as part of their assigned primary group. Cert Rep observed 14 toddlers children receiving care in the classroom, therefore leaving two children not identified as being in an assigned primary group.

At 11:30 AM, 15 children were observed receiving care in the Right-Side Classroom 1 childcare space that has a designated maximum capacity of 14.
Upon review of the facility's Fire Drill log, the following drills were observed to have been conducted more than 60 days apart: 3/18/24-6/3/24 (77 days).
Upon review of the facility's 30-day fire testing documentation, the following tests were observed to have been conducted more than 30 days apart: 11/22/23-2/16/24 (86 Days), 2/16/24-4/4/24 (48 Days), and 4/4/24-7/2/2024 (89 Days).
The files for Staff #1, #2, and #4 contained disclosure statements that lacked witnesses. The file for Staff #1 contained an NSOR Certificate that was obtained after the 45-day provisional period; therefore Staff #1 was incorrectly provisionally hired as they were allowed to work past the 45 days without the complete NSOR Certificate. The file for Staff #2 contained complete clearances at the time of inspection, it was observed that the file for Staff #2 lacked an FBI Clearance request dated prior to the date that Staff #2 began working with children. Therefore, she was incorrectly provisionally hired.

In Right-side Classroom 2 there was a cracked yellow bin and a blue couch that had ripped and exposed wood posing a pinching and splintering hazard to the children in care. In the Outdoor PlaySpace there were three cracked plastic puzzle pieces that were cracked on the corner posing a cutting hazard the children in care.
The file for Staff #1 contained Tuberculosis Screening at the time of inspection, but it was observed they did not have Tuberculosis Screening before their first day working in childcare.
Although Staff #2's file contained Pediatric First Aid/CPR it was observed that Staff #2 had not completed the following required pre-service training within 90 days of their date of hire (see LIS code sheet): (Pediatric First Aid/CPR).
Staff Person #4, qualified to be an Aide, was observed to be unsupervised, caring for children in the PreK Classroom.
Observed hand sanitizer that was attached to a staff members bag that was in a low unlocked cabinet and accessible to the children in care.
The first aid kit located in the Left Side Classroom lacked tape.
The file for Child #3, a preschooler, lacked an updated health report within the last 12 months.
The Health Report for Child #4 lacked documentation from the physician, physician's assistant or a CRNP that included a list of the child's current medication, the reason for the medication, and if a special diet is prescribed for a child.
The files for Child #1 and #3, enrolled for more that 60 days at the facility, did not contain documentation of an influenzas vaccination as required based on the child's age as per the ACIP recommended schedule.
The Health Report for Child #4 lacked a statement indicating the child is able to participate in child care and appears to be free from contagious or communicable disease.

There was a foam pumpkin in Infant Room B that had multiple bite marks on it. It was accessible to the children in care who are still placing objects in their mouths. Observed multiple foam pumpkins in Infant Room A where the material had come loose, and the foam was exposed. These pumpkins were accessible to the children in care who are still placing objects in their mouths.
The Emergency Contact Form for Child #2 lacked the health insurance coverage policy number for the child.
The Health Report for Child #4 lacked documentation from the physician, physician's assistant or a CRNP that included a review of the child's history.
Observed shaving cream labeled "Keep out of Reach of Children" in the cabinet in the Infant B Classroom. Staff stated that it was used for art with the children.

The Health Report for Child #4 lacked documentation from the physician, physician's assistant or a CRNP that included an assessment of an acute or chronic health problem or special need and recommendations for treatment or services, including information regarding abnormal results of screening tests for vision, hearing or lead poisoning.
Although the file for Staff #1 contained a complete FBI Clearance at the time of inspection, it was observed that the file lacked fingerprinting dated prior to the date that Staff #1 began working with children. The file for Staff#1, employed more than 45 days, lacked a NSOR Certificate at the time of Renewal Inspection. Therefore, she was incorrectly provisionally hired. Although the file for Staff #2 contained complete clearances at the time of inspection, it was observed that the file for Staff #2's lacked an NSOR certificate and Child Abuse Clearance within 45 days of hire. Therefore, she was incorrectly provisionally hired.
In the Toddler Section of the outdoor play space, the surface of the structure was peeling and flaking, creating a tripping hazard and rough edges.
Observed in the Infant 2 Classroom that the door to the restroom was blocked due to the stacked rest equipment.
Observed a loose table leg creating a pinching hazard for the children in care in the Right side 2 Classroom.

In the Left Side Classroom, staff was observed changing a toddler's diaper. Staff did not ensure the child's hands were washed after being diapered.

Based on Fire Drill documentation, fire drills are not being held at least every 60 days; 8/31/21, 11/4/21, 2/28/22, 4/15/22, 6/23/22, and 8/25/22.
In the Outdoor PlaySpace there was a broken and cracked toy truck that had rough edges. In the Outdoor PlaySpace there was a Playhouse that had a cracked water fountain that had sharp edges.

In the 1 year old room, there was a circular fabric climbing structure that had tearing in multiple areas with small pieces coming apart that were less than 1 inch in diameter that could be placed in a 1 year old child's mouth.
