Facility Person #1 left the classroom with the children and was not accompanied by any other staff person at this time. On 4/29/25 video footage of the K-Prep room from 4/25/25 at naptime was reviewed. In the video, it was observed, and confirmed by staff statements, that Facility Person #2 was the only staff person in the classroom supervising the children. Review of staff files on 4/29/25 indicates there was no documentation on file that Facility Person #1 or Facility Person #2 had completed required approved pediatric first aid & CPR training. According to staff statements, they have not yet completed the training. On 4/29/25, facility administrative staff were notified that Facility Persons #1 and #2 were required to be supervised at all times by at least an assistant group supervisor who has completed all required pre service trainings. Staff #2 placed Child #1 on the floor in a sitting position. Staff #2 then moved to remove bibs from other children in the room and was not aware that Child #1 had fallen over and bumped his head and face on the floor. (See LIS Code Sheet)
In the infant room, a staff person, after changing a diaper, was observed first unlatching the lock on the lid with their hand to discard a diaper, and then used their foot to open the hands-free diapering can.

An adult health assessment must include tuberculosis screening by the Mantoux method at initial employment. Subsequent tuberculosis screening is not required unless directed by a physician, physician's assistant, CRNP, the Department of Health or a local health department.
