At the time of the inspection, staff person #15 only completed 1 clock hour of child care training during the 1/24-1/25 training period. Staff person #16 only completed 11 clock hours of child care training during the 12/23-12/24 training period. Staff person #17 only completed 6 clock hours of child care training during the 4/24-4/25 training period. Staff person #18 only completed 6.5 clock hours of child care training during the 11/23-11/24 training period. Staff person #19 only completed 3.5 clock hours of child care training during the 12/23/12/24 training period. Staff person #21 only completed 7 clock hours of child care training during the 4/24-4/25 training period.
At the time of the inspection, there was no documentation of emergency plan training on file for facility person #3. Staff person #20 did not have updated emergency plan training on file for when the plan was updated in July 2025. This is evidenced by the emergency plan training on file dated 12/16/24.
At the time of the inspection, there was no current child service report available for review for child #1.
At the time of the inspection on 8/29/25, facility person #1 did not have a current PA State Police clearance on file. This is evidenced by the PA State Police clearance on file dated 8/10/20. The Director acknowleged that staff person #1 had routine interaction and direct contact with children during 8/10/25 - 8/21/25, prior to being removed from the position.
At the time of the inspection, child #5 was not in compliance with the immunization recommendations according to the ACIP and was missing: HEP #2 and #3, DTAP #4, Hib #2 and #3, HEP A #1 and #2.
At the time of the inspection on 8/29/25, the annual inspection report of the interconnected fire detection system dated 8/1/25 showed deficiencies and was not in compliance with the standards established under section 1016(c) of the act (62 P.S. § 1016(c).
At the time of the inspection on 6/27/25, the Director acknowledged that the fire detection system is set off during each fire drill; however, the manual monitoring of the system has not been conducted every 30 days as required. This is evidenced by the current manual monthly monitoring date of 6/25/25 and the previous date of 5/21/25, where the fire detection system was manually tested.

The emergency plan did not include continuity of operations during and after an emergency that includes continuing daily operations, backing up or retrieving health and other key records/files and managing financial issues such as paying employees and bills during the aftermath of the disaster.
The emergency plan did not include a method for facility persons to contact parents as soon as reasonably possible when an emergency situation arises or a method for facility persons to inform parents that the emergency has ended and to provide instruction as to how parents can safely be reunited with their children.
STAFF #2 fire safety training was dated 5/24/22.
STAFF # 1 (see code sheet for start date) health assessment was dated 6/23/21. An updated health assessment dated 8/8/23 was turned in on 8/9/23. Updated health assessment was completed 46 days after the previous one expired.
STAFF #3 completed the one-hour 2022 update to the health and safety training on 11/15/22. STAFF #3 did not complete the initial health and safety training. STAFF # 4 pediatric first aid & CPR is dated 4/7/21. The expiration was 4/2023.
Staff person #-1-2-3-4-5-6-7-8-9-10-11-12-13-14-15-16-17 & 18 have acceptable pediatric first aid/CPR training in an approved curriculum; however, the trainer has not provided documentation that they are PQAS-certified in that training topic.
Child number 1 start date 3/15/22 does not have a record of immunization.
During inspection on 9/28/21 operator was not able to locate inspection summary dated 8/11/21. This summary contains a regulatory noncompliance that has not been corrected.
During inspection on 8/11/21 it was revealed that the annual facility fire safety test had a less then passing grade, the test included failed items without a history of correction.
At the time of the renewal inspection, the facility was unable to provide proof of date of purchase and installation of fire detection system as required by section 1016(c) of the act (62 P.S. §1016(c)).
Staff person #11 did not have an annual evaluation on file. This is evidenced by the only evaluation dated 7/14/23 on file.
During the inspection, the operator admitted to not providing the daily schedule, hours of operation, fees and supervision policies to families at the time of application.
The health report in the file for child #1 did not include a statement that the child is able to participate in childcare and appears to be free from contagious or communicable disease.
At the time of the inspection, volunteer #19 did not have a health assessment or TB screening at initial service in a childcare setting (see LIS code sheet for initial service in a childcare setting)
At the time of the inspection, volunteer #19 did not have two written, nonfamily references from individuals attesting to the person's suitability to serve as a facility person.
During the inspection, the annual fire inspection report dated 7/31/24 from Johnson Controls showed deficiencies relating to Panel functional failure in the main switchgear room and the pull station located by the St Lukes office area.
During the inspection, the operator admitted to not providing Chapter 3270 regulations electronically to parents.
The emergency plan did not include accommodations for children with special needs. The letter provided to parents does not include accommodations for children with special needs as well as all required emergency procedures.
At the time of the inspection, staff person #2 did not obtain NY clearances at time of hire and also resided there in 2018 (see LIS code sheet for date of hire). Volunteer #19 did not have a request for or completed PA State Police clearance or request for or completed NSOR certificate. There was no signed disclosure statement either for volunteer #19 on file at the start date of volunteering (see LIS code sheet initial service in a childcare setting).
The written plan identifying the means of transporting a child to emergency care and staffing provisions in the event of an emergency was not posted in Class 24 or the Starlight room.
During the inspection, safe routes were not posted in the facility.
