On 02/24/26, during the renewal inspection, the 1st aid kit in classroom #3 did not contain gauze, the 1st aid kit in classroom #5 did not contain tweezers, and the 1st aid kit in classroom #10 did not contain scissors.

On 02/24/26, during the renewal inspection, the toilets in classroom #10 and the toilets in classroom #7 did not contain signs which stated that a facility person and an able child shall wash their hands after toileting and before eating.
On 10/07/25, a video of staff person #1 threatening to harm a child, was captured. On 10/17/25, during an interview, staff person #1 admitted to threatening to harm a child by saying "I am going to sit on you."
On 09/15/25, during the complaint investigation, the Procare reports that on 09/04/25 the diaper of child #1 was checked and changed at 9:03AM, 12:15AM, 2:50PM. On 09/04/25 the Procare reports that the diaper of child #1 was changed at 9:17AM and 12:00PM.
On 06/02/25, during the complaint investigation, it was confirmed that after lunchtime, Child #1 was crying because they wanted their cup. Staff person #1 picked Child #1 up by their right arm in order to move child to their cot because child was upset and unwilling to walk to their cot after being directed verbally.
On 06/02/25, during the complaint investigation, it was discovered that staff person #1 was squirting children in the face with a squirt bottle while children were toileting to make them go to the bathroom and to get them used to "feeling wet".
On 06/02/25, during the unannounced monitoring, child #1 in infant room 1 was observed to be sleeping in a bouncer at 10:45am. It was reported that the child had been asleep in the bouncer since 10:12.
During the unannounced inspection on 06/02/25, the staff in classroom D3/pre-k, early pre-school, PPB, preschool 2, preschool 1, toddler 2 and pre-k 2, did not know the names and whereabouts of the specific children assigned to them for supervision. When asked staff had difficulty identifying their specific group of children assigned tot hem for supervision. The staff people in the toddler 2 and pre-k 2 classrooms stated that they were not assigned responsibility for the supervision of specific children.

At the time of the inspection, Staff 1 and Staff 2 did not have proof of experience.
At the time of the inspection, Staff 2 did not have proof of a current CPR training certificate and was past their 90 days.
At the time of the inspection, Staff 3 did not have the results of the TB test on file.
At the time of the inspection, Staff 1 and 2 did not have proof of Pediatric CPR and First Aide on file and was past 90 days from hire.
AT the time of the inspection, staff 4 did not have proof of completing the Mandated Reporter training with in the 90 days of their start date (see LIS code sheet).
On 03/30/23, A facility child hit staff member #1 on the arm, and staff member #1 hit the child back on child's arm. Staff member #1 also squeezed a facility child's cheeks because the child was hitting child's arm on the table while awaiting breakfast.
During the renewal inspection on 03/27/23, Certification Representative reviewed the emergency plan and found that the plan did not provider for accommodations for infants, toddlers, children with disabilities, and children with chronic medical conditions.
Staff person #2 does not have a State Police clearance on file. Staff person #4 does not have out of state clearances on file. Staff person #5 does not have a NSOR clearance on file. Staff person #6 does not have a DHS FBI clearance on file.

Facility self-reported that on 7/16/21, around 9:54am, the Early Preschool 2 classroom (older toddlers) left their room to go to outside play space. They normally go outside through the PPB classroom but because they were doing water play, they were going out through another door. Staff person #1 was in the front of the line and saw three children go into the PPB classroom which is their normal routine. She told staff person #2, at the back of the line to go get them. She did not stop to do another name to face check, nor did staff person #2 retrieve all the children, only one child. Both staff failed to do name to face checks at multiple transition areas on the way to the playground as required by facility procedure. The children were found within 2 minutes by another facility person and the director.
