A plastic green basket containing linking logs and a plastic blue basket containing assorted wood shapes were observed on a wooden shelf in the Pre-K C115 Room. Both baskets were cracked and posed a potential scratching hazard.
Miracle-Gro potting mix was observed in a metal drawer in the Pre-K C115 Room. This potting mix was labeled, "keep out of reach of children," but was accessible to the preschoolers being supervised in that room.

An outlet wall plate cover, near the exit to the Gym, was observed to be cracked and had sharp edges. Also, part of the metal fencing, that was previously attached to the wooden fence in the Outdoor Play Area, was observed to be bowing downward and hanging loosely.
The file for Staff #3 was observed to be missing one written, nonfamily reference from an individual attesting to the staff person's suitability to serve as a staff person.
The facility had documentation on file showing that they notified the local traffic safety authorities of the location of the facility and the program's use of pedestrian and vehicular routes around the childcare facility. The most recent documentation on file was dated 9/12/24. However, this was required to be done annually.
The emergency contact information on file for Child #1 was observed to be missing the address of the individual designated by the parent to whom the child may be released.
On 6/26/2025, at approximately 1:08 p.m., Child #1 was left unsupervised in the hallway outside of the Preschool Room (C116), for approximately 30 minutes, when Child #1 exited the Preschool Room (C116) alone. Staff #1 and Staff #2 were supervising children together in the Preschool Room. However, Staff #1 was responsible for this child at the time of the incident and was unaware the child left the classroom. The child was found by a non-staff maintenance worker and returned to the program. This incident was self-reported to the Regional Office on 6/27/2025 and Staff #1 was terminated by the facility on 7/7/2025.
A jungle gym that was located in the outdoor playground was observed to have steps, that were about knee height, with peeling plastic and rust.
The sink areas of the boys' and girls' restroom were observed to have a trash can that was missing a lid.
The file for Child #1 did not have signed parental consent for emergency medical care or the administration of minor first-aid procedures by facility staff.
Staff person #4 did not have any education on file to determine whether or not the staff qualifies for the position that they are performing.
A renewal inspection was conducted on 1/22/24. In the classrooms, the evacuation routes were not posted in a conspicuous location. The director acknowledged that the posting must have been removed.
