While at the facility unannounced on 4/27/23 for the renewal inspection the inspector observed staff #1 working directly with children at the center. The file for staff #1 was subsequently reviewed and the file for staff #1 did not have a completed FBI clearance nor a completed Pa State Police clearance on file at the facility. This staff has been working over the 45-day provisional hire period.

Cert rep made an unannounced complaint visit to the facility on 7/16/21. The facility was out of ratio when Cert rep. arrived at approximately 3:00pm. Facility person #1 stated they were following nap-time ratios however, most of the children were awake and moving around the room while a few of the children were napping. Facility person #1 stated she was supervising 8 preschool children and 1 young toddler in her assigned group. Facility person #2 stated she had 5 preschool children, 2 older toddlers and 3 young toddlers in her group. Facility person #3 had 3 infants in her group and was in ratio. During Cert rep's visit 6 children were picked up and staff were able to come into ratio with moving children into different groups within about 15 to 20 minutes.
