Maintain a written emergency plan in a readily accessible location that includes all required components.
Ensure bedding is stored so that there is no contact with the sleeping surface of another mat throughout the center. This was corrected onsite.
Provide a current life/hazard use registration certificate applicable to the center's licensed capacity and ages served.
Ensure that diapering surfaces are flat, smooth, clean, dry, non-absorbent and in good repair in the bathroom near classroom A.
Ensure fire drills are conducted during each session provided at the center, one fire drill per year is conducted during nap time, if applicable, and all children are evacuated from the building within three minutes during each fire drill.

Submit documentation of the annual orientation training for all staff.

COVID-19 Child Care Standards Pursuant to Executive Order No. 149: Ensure that information collected during the entry screening process is recorded on OOL’s Daily Log of Entry Health Screenings and Attendance Form and that staff, children, and visitors are denied entrance into the facility, as required. At the time of inspection the Health Screening and Attendance Form was not used to log the visiting DCF Licensing Inspector.
Submit documentation of 2 staff, present at the center at all times, who are currently certified in CPR and First Aid.

Ensure that the center's fire protective systems are operative at all times. At the time of the inspection, the fire panel in the front of the building was in trouble mode. A call to the monitoring company could not confirm a signal. Fire official was called and met inspectors at the location. Fire official stated the main fire panel in the basement was operating normally.
Ensure that there is at least one staff physically present in the room supervising at all times. Staff in Room B exited the classroom to wash her hands across the hall leaving the children unsupervised.

Ensure that infants are provided age-appropriate, supervised tummy time at least twice per day. Tummy time was not provided in Room A.
Ensure that children are provided opportunities to leave their sleeping equipment to crawl, walk and play. Children in Room A were not provided an opportunity to leave the equipment to explore the classroom as they were moved between the seats, bouncy chairs, highchairs and sleeping equipment.
Ensure that the use of infant equipment, including swings, exersaucers and bouncers, is limited to 30 minutes at a times. Infants in Room A were observed in seats and bouncers for over 30 minutes.
Ensure that each child's sippy cup is labeled with the child's name throughout the center.
Ensure that staff wash their hands with soap and running water immediately after wiping children's noses in Room A.

Ensure that adequate supervision is maintained at all times throughout the center. Staff in Room B were unable to maintain adequate supervision and did not intervene in a timely manner when the following occurred: 1) Children were running throughout the classroom and hitting one another during transition times; 2) Children were hitting and pushing one another while lining up in the hallway to go outside.

Ensure that the center complies with the licensed capacity of the classrooms at all times throughout the center. There were 14 children present in Room B, which has a licensed capacity of 10 children.
Ensure that all staff, the sponsor and/or the sponsor representative complete a CHRI background check as required. Pertains to the sponsor and 1 staff.
Ensure that the center is free of infestations and maintain documentation on file. Rodents were present on the play area where there are open drain pipes and cracks in the walls.
Ensure that all toxic substances are inaccessible to the children. Diaper creams were accessible on the low shelf of the diaper changing station in Room A.

Ensure that the center has an adequate supply of materials to meet the daily activity needs of the children and the program. Ensure Room A is supplied with a minimum of 4 learning areas with 4 different activities in each.
Ensure that the center completes and maintains on file a Staff Records Checklist designated by OOL.
Ensure that both staff-directed and child-selected activities are provided throughout the center. Children were engaged in child-selected free-play without opportunities for staff directed activities in Rooms A and B.
Ensure that a minimum temperature of 68 degrees Fahrenheit is maintained in all areas used by the children. Temperatures were not maintained throughout the center as the boiler was broken.
Ensure that adequate supervision is maintained at all times throughout the center. Staff was unable to maintain adequate supervision and did not intervene in a timely manner when the following occurred in Room B: 1) Children were running throughout the classroom; 2) Children were rolling on the floor, kicking and wrestling.
Provide children under 12 months of age with cribs or playpens in Room A. Infants were observed sleeping in bouncy seats as not enough cribs or playpens were provided in the room.
Ensure that staff know the ages of the children in their care so they can ensure adequate staff/child ratios are maintained at all times throughout the center. Staff in Room A were unaware of the children's ages and could not determine if they were maintaining staff/child ratios.
Submit a copy of the center’s current Life Hazard Use Certificate of Registration applicable to the center’s licensed capacity and ages served.
Provide 2 staff present in the center at all times when 6 or more children are present. There was 1 staff alone in the center with 7 children at the time of inspection.
Ensure staff/child ratios are maintained at all times, there were 3 children, ages 0-18 months, and 4 children, ages 2.5-3 years, present in Room A with 1 staff. An additional staff was required.

Ensure a director designee is present at the center at all times when the director is not present, and that the designee does not have full time classroom responsibilities. At the time of inspection the director was not present, there was no designee, and the only staff present in the building was directly overseeing a class of 7 children in Room A.
Submit the center’s table of organization indicating lines of authority, responsibility and job descriptions of current staff and submit required documentation for any credentialed staff on the table of organization that have not been approved in NJCCIS.
Submit an updated and completed Staff Records Checklist using the most current form.
