The licensing coordinator's request to review documentation of rabies vaccines for animals in the program revealed that there was no such documentation onsite for the animals belonging to the Site Director, who are often present at the program. Corrective action: To correct this action, I have posted their rabies vaccination certificates in my office on the wall. They are now readily available to anyone who may ask to see them.
REPEAT CITATION PREVIOUSLY CITED ON JULY 5, 2023, AND OCTOBER 28, 2024. The licensing coordinator's review of department and program records revealed that Staff F did not submit for a background record check prior to employment, and was present at the program during April vacation. As a result, Staff F had not been determined eligible by the department. Corrective action: To correct this action, a Household Personnel Form is being submitted to DHHS for reviewing so Staff F can continue working for our childcare facility. In the future a stricter plan is in place when completing the hiring process, so another staff member isn't missing a background check before starting in our childcare programs. I will look to my staff roster to see that their name is on it and they are eligible before allowing them to work.
The licensing coordinator observed a large hole in the playground fence, which opened to a sloping hill with rocks, causing potential for injury to children. Corrective action: To correct this action, I will find a company to fix the hole in the fencing area to make it safe for children to play on our playground again, I have also blocked the hole for now and the children are playing on out basketball court going forward, until the hole is completely fixed and deemed safe for children.
Statements made by Staff A, B, C, D, and F confirmed that, on occasion, staff and children have reported animal feces on the playground and inside the building. Corrective action: To correct this action, we have implemented a stricter plan on checking over the playground before childcare programs begin. The dogs are also never allowed alone on the playground and are always supervised to make sure we don't miss any feces.
REPEAT CITATION PREVIOUSLY CITED ON NOVEMBER 3, 2022. The licensing coordinator's review of five staff files revealed that one staff had not completed the required 18 hours and one staff had not completed the required 12 hours of professional development during the previous calendar year, as determined by their position and hours of employment. The licensing coordinator's review of the staff files further revealed that one staff had not completed the required Health and Safety training, and one staff had not obtained CPR and First Aid certification within 90 days of hire. Corrective action: The correct this action, staff will work monthly on their required hours for professional development. This will help them to stay up to date on their yearly 18 hours of PD. Staff have already completed their missing hours and are up to date.
The licensing coordinator's review of program records revealed that staff are not reviewing the EOP plan at least four times per year. Corrective action: To correct this citation we have posted our records of dates and times our staff review the EOP throughout the year. We will continue to review and improve our EOP plan and make sure all new staff are reviewing it as well. We will also update our posted records through out the year as needed.
The licensing coordinator's review of program attendance records revealed that five-year-old kindergarten children were attending the program, when the program is only licensed to care for children age 6 and older. Staff A stated that there were five-year-olds enrolled in the program. Corrective action: We requested letters from the two individuals whom completed the Health Inspection and Life Safety and Compliance Report back in October of 2022 that stated they were okay with our organization changing our license minimum age to 5 years old. Both individuals completed letters stating they were okay with us revising our license to 5 years of age to 15 years of age. We also completed a revision application that was submitted yesterday, September 25th, to our Licensing Coordinator so that our License can be revised to accept 5 year old Kindergarten students.
REPEAT CITATION PREVIOUSLY CITED ON NOVEMBER 3, 2022. The licensing coordinator's review of five staff files revealed that one file did not contain a written record of a health screening for a staff who had been employed for more than 60 days. Corrective action: To correct this citation we have reviewed all employees Health screenings and double checked all files to make sure every employee is compliant with health screenings. All employees at this time have up to date health screenings on their files and are healthy to work with children.
The licensing coordinator's review of five staff files revealed that they did not contain all required documentation. Corrective action: To correct this citation we have reviewed all files of staff currently working and recorded what they have and what they are missing to comply with DHHS requirements. All staff are now currently up to date on all documentation required by the DHHS.
REPEAT CITATION PREVIOUSLY CITED ON DECEMBER 27, 2021 AND JULY 5, 2023. The licensing coordinator observed one staff in the program who had not been determined eligible by the Unit. The licensing coordinator reviewed a current staff list provided by the program and determined that four staff did not submit for a background record check prior to employment. Corrective action: To correct this we have gone through all employees who had not submitted a BRC application after completing their BRC & fingerprinting. We are having them all complete their application before continuing work. We will have each employee who completes a BRC & fingerprinting to return to the childcare facility that day to complete their application as well so they are eligible to work through the state of NH.
This Statement of Findings includes repeat citations and failure to comply with approved Corrective Action Plans for He-C 4002.09(b) and He-C 4002.33(f), as noted herein. Corrective action: To correct this, we will comply with the corrective action plan and work to correct our citations immediately. We have created a new way of filing and organizing our file system for employees and new hires. This will allow us to keep up on all required documentation and avoid any citations in the future.
