The program shall request and maintain on file for each child documentation of immunizations in accordance with RSA 141-C:20-a, RSA 141-C:20-b, and He-P 301.14.

The licensing coordinator’s review of nine children’s files revealed that there was no record of a health screening on file within 60 calendar days of the date of admission and no record of immunizations on file for one child. Corrective action: The childcare staff will complete a monthly audit of all student files to ensure that emergency contacts are present for all children and secure all necessary information for any children without emergency contacts.

Prior to employment or residency, as applicable, and every 5 years thereafter, all child care staff, substitutes, other employees, and volunteers who may be alone with children or are included in staff to child ratios, and household members age 10 years or older, shall submit for a background record check using NH Connections.

The licensing coordinator reviewed program records and determined that one staff did not submit for a background record check prior to employment. The program confirmed this individual did not submit for a background record check prior to employment. Corrective action: As of December 19, 2024, the staff identified are no longer permitted on the premises of the program and will not return until they are determined eligible to work by the Unit. All staff will submit for a background record check prior to employment and every 5 years thereafter, and will not be on the premises until notification from the Unit that they may work under supervision or until they are determined eligible, as applicable.

Child care staff shall ensure that for each child, upon the child’s first day in attendance in the program, there is a “Child Care Registration and Emergency Information” (April 2022) form or an equivalent form provided by the child care program completed and signed by the parent, on file, which contains: 1) Full legal name of the child; (2) Child’s date of birth; (3) Child’s physical address and mailing address; (4) Child’s home telephone number; (5) Date of enrollment in the program; (6) The name, physical address, and mailing address of the parent(s) responsible for the child, if different from the child’s address; (7) Telephone numbers for the child’s parents and instructions as to how the parents can be contacted during the hours that the child is at the program; (8) Email addresses for the parents, if available; (9) Names and telephone numbers of at least one person who will assume responsibility for the child, if for any reason, the parents cannot be reached immediately in an emergency; (10) Any chronic conditions, allergies, or medications in case of sudden illness or injury; (11) Written parental permission for first aid treatment; (12) Written parental permission for emergency medical transportation and treatment; (13) The name and telephone number of each child’s physician or health care provider; and (14) Names and telephone numbers of any person(s) other than parents who are authorized to remove the child from the program.
