The licensing coordinator observed that a crockpot was being used to heat bottles in the Butterflies classroom. Corrective action: The staff ceased use of the crockpot at the time of visit and a waterless bottle warmer is now being used.
REPEAT CITATION PREVIOUSLY CITED ON NOVEMBER 28, 2023, AND NOVEMBER 26, 2024. The licensing coordinator observed a bag of googly eyes in an unlocked drawer measuring 29 inches from the floor in the Bumblebees classroom, where children under three years of age are present. The staff immediately moved them out of reach. This constitutes the second repeat citation and failure to comply with two approved corrective action plans regarding access to materials that pose a suffocation or choking risk or are small enough to be swallowed. Corrective action: Child safe locks have been installed on all drawers and cabinets in every classroom. Monthly checks will be completed by maintenance to ensure all locks are in safe working order. This will ensure that children do not have access to small items no matter where they are stored in the classroom.

The licensing coordinator's observation that the refrigerator in the Caterpillar classroom was missing a thermometer, which prevented staff from ensuring the storage of food was consistent with safe food handling practices. Corrective action: Thermometer has been placed and monthly thermometer checks will have been added to the kitchen managers checklist.
The licensing coordinator observed children sitting at a table during a teacher-directed activity using small beads, supervised by staff in the Bumblebees classroom. The staff members stated that there are children between the ages of 24 months and 3 years enrolled and present in the classroom. The program manager stated that the program has not obtained written parental or guardian consent acknowledging their understanding that the program uses small items during teacher-directed and supervised activities. Corrective action: A parent/guardian permission form for small item use under direct teacher supervision has been created and signed by the parents of all currently enrolled children ages 24 months to 3 years of age. The form has been added to the enrollment packet for all future child enrollments to be completed prior to starting.
The licensing coordinator's review of attendance records and registration forms in the Butterflies classroom revealed that three children started prior to turning six months, when the program license permits enrollment of children between six months and five years of age. Corrective action: No children will be enrolled prior to 6 months of age.
The licensing coordinator's review of 12 children's files revealed the registration and emergency information on file was missing the required statement under He-C 4002.10(c). Corrective action: Updated forms with the required statement have been distributed to all current families to update files.
The licensing coordinator's review of the daily attendance for March 12, 2025 revealed that Staff E and Staff I did not sign in using their full names. Additionally, Staff B, Staff F, Staff G, and Staff H were signed into the Caterpillars classroom, when they were not present, therefore not documenting the daily attendance in real time. Corrective action: Proper protocol for signing in/out in real time was reviewed with the direct staff involved with this incident and then with all staff at a team meeting. that took place on 3/19/25. Reviewed protocol included signing in/out in real time, including when leaving the room for breaks and scheduled planning times as well as documenting full name (first & last) of staff and/or children who are handwritten as additions on the daily attendance sheet. This protocol will be added to our onboarding process as well as our daily attendance checks.
The licensing coordinator's observation that there was of a baggie in the Grasshoppers classroom containing a crushed up pill, and there was no label or medication order from a licensed professional to identify the content, the doseage, or to whom the medication belonged. Corrective action: In the future, proper medication protocol will be reviewed with staff. Prescription medications will be reviewed for compliance by the Center Administration. This will be reviewed upon hire as part of our onboarding process. All classroom staff will be required to complete a monthly medication check that is turned into Administration. They will list all medications that have been accepted and sign off that all regulations are being met.
REPEAT CITATION PREVIOUSLY CITED ON NOVEMBER 28, 2023. The licensing coordinator's observation that there was an expired Epipen and Benadryl that belongs to an enrolled child in the Grasshoppers classroom. This constitutes the first repeat citation and failure to comply with one approved Corrective Action Plan regarding expired medications. Corrective action: In the future, a medication date check and sign-off will occur the first week of every month to ensure all medications and medical orders are current. No children were in any danger.
This licensed complaint visit inspection recorded no violations or advisories.
The licensing coordinator's review of program records revealed that one staff present at the time of the visit, had not completed the background record check process and was not eligible to work. Corrective action: No children were in danger and this employee was not left alone unsupervised. The employee was fingerprinted in January 2023 once she turned 18 but the BRC process was incomplete in the NH Connection System. Now and in the future, our agency will double check to make sure the process is complete in the NH Connection System.
The licensing coordinator's observation revealed that there was a bin of small counting bears and a bin of popcorn kernels stored in a cabinet with a broken lock in the Caterpillar room, making them accessible to children under the age of three. Corrective action: No children were present in the classroom during the visit. No children were harmed or in any immediate danger. Now and in the future, our agency will do monthly checks all child safety locks and devices.
The licensing coordinator’s observation of milk beyond the recommended date of use in the refrigerators of the Bumblebees and Grasshoppers classrooms. Corrective action: In the future, the expiration dates on milk stored in classroom refrigerators will be checked at the beginning of each day to ensure they have not expired. No children were in any danger.
REPEAT CITATION PREVIOUSLY CITED ON September 28, 2022. The licensing coordinator's review of Emergency Operations Plan (EOP) drills revealed that the program conducted one component of their EOP in the 2023 calendar year. This constitutes the first repeat citation and failure to comply with one approved Corrective Action Plan regarding practicing components of the EOP each calendar year. Corrective action: In the future, the Fire Drill & Emergency Drill log form will be utilized for all required EOP and fire drills. This will serve as a better notice for upcoming drills. No children were in any danger.
The licensing coordinator's observation that the water temperature in the hand washing sinks measured higher than 120 degrees in the Butterflies and Inchworms classroom and in the hallway bathroom on the second floor. Corrective action: In the future, water temperatures will be adjusted and monitored not to exceed 120 degrees. No children were in any danger.
REPEAT CITATION PREVIOUSLY CITED ON SEPTEMBER 28, 2022 AND NOVEMBER 28, 2023. The licensing coordinator's review of the daily attendance records revealed that there were three staff present and only one staff signed into the Bumblebees classroom, and that one staff was signed in and out of the Inchworms classroom prior to their shift ending, when signing in and out in real time is required. This constitutes the second repeat citation and failure to comply with two approved Corrective Action Plans regarding staff attendance records. Corrective action: In the future, attendance recordings will be reviewed with all staff. Center Administration will monitor daily for compliance by visiting each classroom and checking that records are complete and accurate. No children were in any danger.
This Statement of Findings includes repeat citations and failure to comply with approved Corrective Action Plans for He-C 4002.08(b), He-C 4002.17(f), He-C 4002.21(u), and He-C 4002.325(f), as noted herein. Corrective action: In the future, all corrective action plans will be added as standing agenda items to our monthly staff and Admin meetings for review and compliance. No children were in any danger.
REPEAT CITATION PREVIOUSLY CITED ON SEPTEMBER 28, 2022 AND NOVEMBER 28, 2023. The licensing coordinator's observation of a bag of one-inch letter blocks and a bag of buttons in an unlocked drawer 25 inches from the floor, and a tray that contained marbles on the lowest shelf of a toy shelf in the Bumblebees classroom. The staff immediately moved these items out of reach of the children. This constitutes the second repeat citation and failure to comply with two approved Corrective Action Plans regarding small parts being accessible to children. Corrective action: In the future, each classroom will have a designated space in a locked closet to store small parts out of reach of children when not being directly supervised by staff (for children age 30 months to 3 years). No children were in any danger.
Two licensing coordinators' observations revealed that one staff present in the Bumblebee room and one staff present in the Dragonflies room had not signed in upon arrival. Corrective action: Our agency staff the classrooms one above state ratio requirements. Now and in the future, all agency staff will sign in and out of the attendance once enter and leaving the room. Program director will monitor daily.
The licensing coordinator's review of program records revealed that the program has not conducted an off-site, bomb threat drill with all staff. Corrective action: Our agency completed 2 our of 4 required EOP drills. One classroom completed the off-site evaluation, not the entire program. Now and in the future, all staff will complete off site evaluation and all EOP components. The agency updated the onboarding process and yearly trainings will include all EOP components.
Two licensing coordinators observed an expired Epi-pen in the Inchworm classroom and one bottle of expired BabyBum sunscreen in the Dragonflies classroom. Corrective action: The epi pen was one week expired and the parent was notified. Now and in the future, all parents will replace emergency medication on or before the expiration date. Staff will monitor this. process Now and in the future, agency staff will require parents to take sunscreen home after the summer season. Staff will be monitored to complete this task.
The licensing coordinator's review of program records revealed that fire drills were not done in December 2022 or January 2023. Corrective action: Due to workforce and illness our agency was not able to complete 2 out of the 12 required fire drills in a year. Now and in the future, our agency will complete all require NH Child Care Licensing drills.
