The licensing coordinator observed two Dial Antibacterial liquid soaps, labeled “keep out of reach of children,” at the handwashing sink used by children in the Preschool classroom. Corrective action: The soap has been replaced with kid friendly soap Going forward the center will only buy and use kid friendly soap for children
Staff members in the Infant classroom stated that they check on sleeping infants every 30 minutes, when staff are required to do in-person checks at least every 15 minutes. Corrective action: All infant staff are retrained on the policy that sleep checks for infants are every 15 mins. The lead teacher will be ensure that checks are being made on napping infants by using the app as a timer to make sure checks are being made Teachers have been reminded of the importance of practicing sleep safe practices. In order to ensure that sleep safe practices are being followed the director throughout the day will do random checks.

The licensing coordinator’s review of the daily staff attendance records for September 22, 2025 through September 26, 2025, revealed that the records did not include staff’s scheduled work hours. In the Older Toddler classroom, there were four occurrences when staff did not sign out of the classroom upon their departure and one staff who was present on September 26, 2025, was not signed into the classroom. In the Preschool classroom, there was one occurrence when a staff did not sign out of the classroom upon their departure. Additionally, staff stated that the daily attendance records do not reflect when groups are combined. Corrective action: The Attendance record form has been updated to include staff hours worked and a place for staff to side in and out including when they take a lunch. Staff have been retrained on the importance signing in and out so that we are in ratio. The director or assistant director will check attendance sheets three times a day to make sure that staff are signing in and out on the attendance sheets When classrooms are combined teachers will sign in and out of every attendance sheet of the children that they are supervising.
The licensing coordinator’s review of five staff files revealed that in 2024, two staff completed 9 ½ hours of professional development within their first twelve months of hire, rather than the 18 hours required by He-C 4002.33(f) of the 2022 rules. Corrective action: The director will oversee the staff professional development training. The teacher will be require to fill out training logs and turn them in to the director the first week of every month along with copies of the training certificates
The licensing coordinator observed that a staff member changing a two-month-old infant’s diaper on an elevated changing surface did not keep one hand on the infant while bending down to collect wipes from a low shelf below the changing surface, or when returning the wipes to the low shelf. Corrective action: All Staff have been made aware of the importance of having one hand on the child when collecting supplies or reaching for something when a child is on a elevated changing table We have posted a reminder at every changing table to ensure the safety of the children.
The licensing coordinator observed one EpiPen without a prescription label or medication order in the Young Toddler classroom. Corrective action: We replaced the EpiPen without a prescription label or medication with EpiPen that had the prescription. Going Forward all new EpiPens will be check by the director or the assistant director to ensure that they have a prescription label or medication plan. The program will not accept any prescription medication that does not include a prescription lablelor medication order from a licensed practitioner.
The licensing coordinator’s observation of a liquid soap dispenser that contained a watery substance. Staff stated that children fill the soap dispensers with water when they are low or empty, therefore there was no liquid soap available and accessible to children and staff in the Preschool bathroom. Corrective action: The staff have filled the soap container and every morning the staff will make sure that they have soap for the children. The director will check the classrooms daily and throughout the day to ensure that children have access to kid friendly soap.
The licensing coordinator's review of 10 child files revealed that none of the children's registration and emergency information forms were the equivalent of the August 2025 form, as the forms were missing the required statement under He-C 4002.10(c). Corrective action: All parents were given an October 10, 2025 deadline to fill out and return to the office. The assistant director is updating the childrens forms to make sure that all registration forms are updated. Going forward all new enrollments will be required to fill out the new registration emergency forms.
The licensing coordinator observed that the changing table in the Older Toddler classroom was soiled, which demonstrated that the changing table was not cleaned on a regular basis. Corrective action: After each diaper change and has need the staff will clean the changing table. The director and assistant director will be spot checking when diapers are changed to ensure that the cleaning of the changing table is being followed. Also all toys, equipment, furniture, and learning materials will be cleaned on a regular basis.
The licensing coordinator observed a staff in the Young Toddler classroom wipe two separate children’s noses without washing their hands in between or after handling the tissues. Corrective action: Staff have been made aware of the importance of washing hands when wiping noses. The director and assistant director will be making spot checks to ensure that hand washing after nose wiping is taking place, Staff will follow the state handwashing polilcy for staff and children
The licensing coordinator observed several prepared infant bottles not labeled with the child’s name and date in the Infant classroom and in the Young Toddler classroom. Corrective action: The staff had been made aware of the policy that infant prepared bottles must be labeled with the child's name and date. Every morning when parents are dropping of their infant child, infant teachers will be checking the infants bottle to make sure that the bottle is labeled with the infant's name and date.
The licensing coordinator observed that in the Older Infant classroom there were six play pens equipped with a loose sheet causing excess fabric and mattresses did not lay flat, both of which posed a potential suffocation hazard to children 12 months of age and younger. Corrective action: I am reaching out to parents to make sure the right pack and play sheets are provided for the pack and plays that we have. I have also purchased new pack and play mattresses. I will also keep extra pack and play sheets on the premises to prevent further situations like this one.
The complainant stated that there is water damage in the preschool bathroom. Staff A stated that there was a leak in the preschool bathroom ceiling that was addressed and fixed, however the ceiling tile was not replaced. The licensing coordinator’s inspection of the preschool bathroom revealed a water stain on a ceiling tile. Additionally, the inspection revealed that the bathroom ceiling vent was visibly covered in dust, the bathroom sink faucet showed signs of irregular cleaning or deterioration as there was black grime around the faucet where it adjoins the sink, and the vinyl wall base molding behind the toilet was pulled away from the wall and the area immediately surrounding the toilet and wall was soiled, demonstrating a lack of regular cleaning. The licensing coordinator’s inspection of the Older Toddler’s bathroom revealed a water stain on a ceiling tile and the bathroom ceiling vent was visibly covered with dust, demonstrating a lack of regular cleaning. Corrective action: I am coordinating with the owners to get the preschools bathroom wall behind the toilet fixed up and the ceiling tiles changed out. I have cleaned the ceiling vents in both classrooms and scrubbed the faucet. We are looking at changing out the faucets as what was noticed does not come off. Moving forward I will work with staff to make sure all cleaning is done properly every day and have them notify me when there are any issues.
The complainant stated that the wood chips in the outdoor play area are sparse. The licensing coordinator’s inspection of the outdoor play area revealed that there was no energy-absorptive material at the base of a climber, measuring 43 inches in height, and around the fall zone, which would allow a child to fall from a height of more than 29 inches and extending 39 inches. Corrective action: The staff will be notified of the closing of said structure and will help keep the children off of it until we get tape to tape the structure off. The staff will stay vigilant in helping make sure even with the tape the children don't climb on it. The tape will not be removed until there is adequate energy-absorptive material underneath the structure. Moving forward we will check our energy-absorptive material regularly to make sure the children are safe, and we stay in compliance.
A review of department records revealed that one staff under the age of 18 years did not submit a household and personnel form prior to employment; one staff did not submit for a renewal prior to their 5 year expiration date; and two staff did not submit for a background record check prior to employment. Corrective action: As of 2/9/2024, all staff under the age of 18 will submit a household and personnel form prior to employment. When a staff member's 5-year expiration is coming up, they will submit a renewal and get a background check before their current one expires to ensure there is no time laps. This also must be done before staff begin employment at It Takes a Village. They must receive notification that they are eligible to work with children before employment.
The program self-reported to the Department that on August 29, 2023, a two-year old child was left outside on the playground for up to five minutes, while the staff and other children were in their classroom. The staff did not notice that the child was missing until an Infant classroom staff found the child outside, in the playground, at the gate upset and crying. Staff A stated that they counted the children while they were re-entering the Older Toddler classroom, and they believed they counted 12 children in line to go inside, but they did not count the children after settling into the classroom. Staff B stated that they did not count the children while they were lining up to go inside and they did not count the children after settling into the classroom. The determination by the department that because staff did not account for all the children during the transition from outside to inside, they inadvertently left a two-year old child outside in the playground unsupervised for up to five minutes. Corrective action: All staff members in the classroom must count when the children line up before going outside and inside. We will have a staff member in the front of the line, as well as the back of the line. There will be a mandatory roll call every half hour whether the children are inside or outside. The classroom teacher is not allowed to close the outside door until all children are counted going in the classroom and then when they are in the classroom again. The teacher must scan the playground before the door is shut. All classrooms will need to post a name to face recognition whether this be on a board or by their hooks. They also will have a copy of this on the back of their clipboard. The clipboard is always accessible and goes outside with them. This will be helpful for any newer staff that may be in the classroom but also when counting the children each time.
The licensing coordinator observed that in the Older Infant classroom there were four play pens each of which were equipped with a soil-stained mattress. Corrective action: I have purchased new pack and play mattresses. I will reiterate to staff that the mattresses need to be cleaned after use and cleaned at the end of every week.
The complainant stated that they are concerned that staff members have not been properly screened, indicating that new staff may not have completed background record checks. A review of department records revealed that one staff under the age of 18 years did not submit a household and personnel form prior to employment. Additionally, two staff present in the program at the time of the visit did not submit for a background record check prior to employment. The director confirmed this information. Corrective action: Following the visit the staff who were noted were asked to leave or not to come in until we were able to resolve this. October 18th we were able to get one application summited to NHCIS and they became eligible the same day. We had submitted another household and personnel form October 16 for personnel under 18. In the future I will make sure all incoming personnel have all the proper paperwork sent in and/or all applications submitted to where they are eligible to work within the center before allowing them to start working.
