The facility had a 100% compliance history prior to this visit. There are fifty-six (56) children enrolled in the facility and thirty-one (31) were present today. The children were observed participating in free play in activity areas and completing bathroom/handwashing routines. The “Annual Compliance Monitoring Checklist for Child Care Centers” was used as a monitoring tool during today’s visit. The staff and training worksheets were received prior to today’s visit. There is a total of seven (7) staff employed. Since the last monitoring visit was conducted, three (3) new staff member has been hired. I monitored the files of the new staff member and the files of one (1) existing staff. I monitored six (6) children's records. I monitored the staff files for the completion of the required health and safety training and current CPR and First Aid certifications. Staff scheduled to work in the infant room have current ITS-SIDS training. I monitored the regulatory system to verify current criminal record qualification letters for all staff. Fire drills and shelter-in-place or lockdown drills have been completed as required. The last fire drill was conducted on 3/16/2026. The last shelter-in-place drill was conducted on 2/19/26. Monthly playground inspections have been completed as required. A superior sanitation inspection was conducted on 6/30/25. The last fire inspection was completed 10/24/25. The Emergency Preparedness and Response Plan was updated in March 2026. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily classroom schedules, activity plans, safe sleep policy, first aid poster, menu, applicable staff/child ratio charts, evacuation routes, infant feeding schedules, cot lists, allergies, and tobacco free signage. There were five (5) violations of childcare requirements observed during today’s visit. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. A baseboard in space #4 was observed in poor repair. 15A NCAC 18A .2825(a) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. Emergency medication was observed in space #4 not in its original container. .0803(2)(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. L. Barrett has First Aid certification on file that has expired. L. MEYER Operation Type: Center Case Number: Visit Date: 12/8/2023 Number Present: 16 Completed Date: 12/8/2023 Age: From 0 To 5 Total Minutes: 89 Time In: 09:41 AM Time Out: 11:10 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. Upon arrival I was greeted by Lisa Morris, Director I shared the reason for the visit. Ms. Morris assisted me with today’s visit. Your program currently operates with a GS-110 Notice of Compliance with day time use only and meeting enhanced space. The center's compliance history was reviewed with the operator. The program’s compliance history was 97% percent as of 12/08/2023. Ownership: The Church is a Non-Profit unincorporated organization. If the church becomes incorporated, you must notify your consultant at least thirty (30) days prior to any changes occurring; this would be considered a change of ownership. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Notice of Compliance, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, New Staff records, Emergency Medical Care Plan, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following items above were found in compliance. The inspections below were found in compliance. •Fire Inspection was conducted on 10/17/2023. •Last fire drill was conducted on 11/30/2023 •Last Shelter in Place/Lockdown Drill was conducted 11/28/2023. •Sanitation Inspection was completed on 6/29/2023 with a Superior classification. •Last Playground Inspection was conducted on 11/28/2023. The children were observed teacher directed art activities, snack time, free center choice time and cleaning up from breakfast. Program Records: We reviewed all the required records and all were in compliance. Staff Records: The staff-training worksheets were completed before the visit. There was one (1) new staff file to monitor. Medication: There was no emergency medication or medication on site to monitor. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. One (1) violation was observed today and was corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In the space that serves two and three year old children there was an electrical outlet not covered and in the area that serves toddlers there was an electrical outlet not covered. 10A NCAC 09 .0604(c) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Technical Assistance •On the playground that serves the older children, consider covering up the MDF board around the bottom of the structure with a boarder to stop the MDF boards from peeling and to prevent any future injuries. •When filling out the Staff Emergency Medical Information form, please ensure that it is filled out in its entirety. •In the building that serves infants and toddlers, part of the boarder around the wall closest to the ground needs to be tacked back into place. It is in an area that is not accessible to the walking children but it’s beginning to peel off in an accessible area for walking children. •There are new NC Sanitation Laws that have been implemented, please reach out if you have any questions.
