Upon arrival I was greeted by Ms. Baker, Assistant Program Director. The administrator, L. McCaskill was not present during. the visit The last annual compliance visit was conducted in March 2025. A walkthrough of the indoor spaces used by children was completed and monitored for the following: supervision, staff/child ratios, CPR and First Aid certification, storage of hazardous products and medication, adequate and approved space, and program records. The staff and training worksheet from the last annual compliance visit was used to monitor for current required information for the existing staff. Two (2) new staff have been hired since the last monitoring visit. The child care consultant monitored the files of the new staff and completed a staff and training worksheet to include the new staff during the visit. All staff have current criminal background check qualifying letters on file. The last documented fire drill was conducted on 1/14/26. A shelter in place drill was completed on 1/14/26. A superior sanitation inspection was completed on 6/25/25 with 0 demerits. The last fire inspection was completed in January 2025. The facility has contacted the local fire inspector regarding having a fire inspection completed before the end of the month. The playground inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, menu, staff/child ratio chart, emergency telephone numbers, emergency procedures form, tobacco free signage and emergency medical care plan. During the visit there were nineteen (19) children present. I observed the children in space #2b during indoor free play. The staff were engaged with the children using positive language and guidance. The children in space #1 were observed transitioning from completing a science experiment in which they made snow to free play in activity areas. The teachers were traveling the room engaging with the children. There was one (1) violation of childcare requirements observed during the visit. Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. M. Gaddy hired 9/2/25 has not completed this required training. .1102(g) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 2/4/26. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during the visit may negatively affect this score. Failure to maintain compliance with our requirements may result in administrative action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Technical Assistance: *Rule Rollout Training in Moodle: The Child Care Commission adopted rules to support QRIS modernization. The new section, .3200, provides the standards for earning a two-though five-star-rated license. To support the QRIS changes, additional rules were amended within the following sections: definitions, developmental day services, and NC Pre-Kindergarten services. Linda McCaskill, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. McCaskill and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member spoke to children in an unkind manner including telling a child that the child needed to be on medication. I met with Linda McCaskill, Center Director, we conducted a walk-through of the facility. The facility operates with a 3-star license that was issued 6/3/22. The facility had a 100% 18-month compliance history prior to today's visit. There are 16 children currently enrolled and 7 were present during the visit. I observed the children present engaged in the following activities: indoor free-play, outdoor free-play, teacher-directed group activities, and bathroom/handwashing routines. Prior to the visit I sent an email requesting the staff and training worksheets to Linda McCaskill, Center Director. The worksheets were received prior to the visit. There have been no new staff hired since the monitoring visit was conducted in November 2023. I monitored two (2) staff records. I monitored four (4) children's records during the visit. The last fire inspection was conducted 2/13/24 and the facility was found to be satisfactory and approved for day time care only. The last sanitation inspection was conducted on 3/5/24. The facility received a Superior rating with 0 demerits. The last fire drill was conducted on 2/19/24. A shelter-in-place and a lockdown drill were completed on 2/28/24. The emergency drill log was reviewed, and drills were documented as required. Playground safety inspections were documented as required. The following items were posted for easy reference: license, Summary of NC Child Care Law and Rules, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, current activity plans, CPR and First Aid posters, menu, applicable staff/child ratio charts, tobacco free signage, and evacuation routes. This facility does not provide transportation. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, program records and management. There was one (1) violation of child care requirements observed during the visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. A fire inspection was completed on 12/12/22 and the next fire inspection was completed on 2/13/24. 10A NCAC 09 .0304(a) The violations documented during the visit must be corrected immediately.
