The last Annual Compliance visit was completed on February 18, 2025. Ms. Stephanie Pearsall, Administrator, was present during the visit. The Annual Compliance Monitoring Checklist was used to denote items monitored during today’s visit. A copy of the checklist was given to Ms. Pearsall at the conclusion of today’s visit. Your program currently operates with a 4-star rated license issued September 5, 2024, with 7 points earned in Staff Education Standards, 2 points in Program Standards, and 1 point in Quality by meeting an education option. Restrictions on the license: Daytime care only and meets enhanced ratios. The NC Secretary of State website was reviewed prior to today’s visit and Foundations Christian Academy, LLC was listed as current-active. I monitored the indoor and outdoor areas during today’s visit. Staff/child ratios, group size, capacity, use of space, and supervision were observed. Children were observed in free play activities and teacher directed activities. All the children were being attended to in a nurturing and appropriate manner and in keeping with their developmental level. Today’s lunch consisted of noodle casserole, green beans, peaches, and milk. REQUIRED INSPECTIONS: -A sanitation inspection was completed on October 13, 2025 with 5 demerits and a Superior rating. -The last fire inspection was completed on January 12, 2025. You stated the fire marshal has been contacted and as soon as an inspection is completed you will forward it to me. -The Emergency Drill Log was reviewed. Prior to today’s visit, the staff and training worksheets were obtained. A review of the information was completed. Four new staff files and five children's records were reviewed. Two violations of child care requirements were observed. Each were thoroughly discussed with Ms. Pearsall. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on 1-13-2025. A current inspection has not been completed. 10A NCAC 09 .0304(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. The administrator and two staff did not complete the Child Maltreatment training within five years of completing the previous training. .1103(b) All violations must be corrected immediately. A compliance letter is due to me within two weeks of today’s visit. By February 24, 2026, please send me a letter addressing how each violation was corrected. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at [email protected]. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history from 8/2/2024 to 2/1/2026 is 100%. Rated License Review- Your current star rate license was effective 9-4-2024; therefore, you are not required at this time to transition to the new QRIS Pathways until September 2027. However, you are welcome to apply at any time. Today, I discussed with you and left a copy of the Facility CQI Plan, as well as a copy of the Individual CQI/PD Plan. You may begin using this at any time for all staff. Prior to today's visit, I verified all teaching staff were in DCDEE WORKS. Maintaining up-to-date WORKS accounts is essential to ensure that staff education records are accurate when you being the application process for the QRIS Pathways. Regularly updating transcripts, credentials, and employment information allows the system to verify qualifications without delay, helping applications move through the approval process more quickly. -E. Lewis is showing up in WORKS but there is no information such as a status letter for her -R.
