The last Annual Compliance visit was completed on February 18, 2025. Ms. Stephanie Pearsall, Administrator, was present during the visit. The Annual Compliance Monitoring Checklist was used to denote items monitored during today’s visit. A copy of the checklist was given to Ms. Pearsall at the conclusion of today’s visit. Your program currently operates with a 4-star rated license issued September 5, 2024, with 7 points earned in Staff Education Standards, 2 points in Program Standards, and 1 point in Quality by meeting an education option. Restrictions on the license: Daytime care only and meets enhanced ratios. The NC Secretary of State website was reviewed prior to today’s visit and Foundations Christian Academy, LLC was listed as current-active. I monitored the indoor and outdoor areas during today’s visit. Staff/child ratios, group size, capacity, use of space, and supervision were observed. Children were observed in free play activities and teacher directed activities. All the children were being attended to in a nurturing and appropriate manner and in keeping with their developmental level. Today’s lunch consisted of noodle casserole, green beans, peaches, and milk. REQUIRED INSPECTIONS: -A sanitation inspection was completed on October 13, 2025 with 5 demerits and a Superior rating. -The last fire inspection was completed on January 12, 2025. You stated the fire marshal has been contacted and as soon as an inspection is completed you will forward it to me. -The Emergency Drill Log was reviewed. Prior to today’s visit, the staff and training worksheets were obtained. A review of the information was completed. Four new staff files and five children's records were reviewed. Two violations of child care requirements were observed. Each were thoroughly discussed with Ms. Pearsall. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on 1-13-2025. A current inspection has not been completed. 10A NCAC 09 .0304(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. The administrator and two staff did not complete the Child Maltreatment training within five years of completing the previous training. .1103(b) All violations must be corrected immediately. A compliance letter is due to me within two weeks of today’s visit. By February 24, 2026, please send me a letter addressing how each violation was corrected. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at [email protected]. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I recommend that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. You are required to maintain compliance with all applicable child care rules and regulations at all times. NC GS 110- 90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Your compliance history from 8/2/2024 to 2/1/2026 is 100%. Rated License Review- Your current star rate license was effective 9-4-2024; therefore, you are not required at this time to transition to the new QRIS Pathways until September 2027. However, you are welcome to apply at any time. Today, I discussed with you and left a copy of the Facility CQI Plan, as well as a copy of the Individual CQI/PD Plan. You may begin using this at any time for all staff. Prior to today's visit, I verified all teaching staff were in DCDEE WORKS. Maintaining up-to-date WORKS accounts is essential to ensure that staff education records are accurate when you being the application process for the QRIS Pathways. Regularly updating transcripts, credentials, and employment information allows the system to verify qualifications without delay, helping applications move through the approval process more quickly. -E. Lewis is showing up in WORKS but there is no information such as a status letter for her -R. This program currently operates with a temporary license effective March 4, 2024, to September 4, 2024. Applicable childcare requirements were discussed with Mrs. Stephanie Pearsall, Owner. Mrs. Pearsall accompanied me as I conducted a walkthrough of the indoor and outdoor spaces used for child care. They are approved to use nine (9) classrooms; however, because of a shortage in staff they are only operating four (4) classrooms. I monitored all indoor spaces approved for child care except for the classrooms that were not open. I monitored the outdoor play spaces. The children observed today, were interacting with assigned staff, playing in free choice areas such as fine motor materials, books, blocks, and dramatic play materials. They were observed resting on individual cots with linens. For lunch the children were served beef ravioli, mixed vegetables, sliced peaches, slice of bread, and milk. During the visit seven (7) staff files, including the administrator’s, and three (3) children’s files were reviewed. In addition, required center program records were also monitored. It is your responsibility to review all files to ensure they are complete and accurate. The following were also observed during today’s visit: staff/child ratios; CPR/FA; storage of hazardous products; adequate/approved space; posted license and permit restrictions. Overall attendance and enrollment were collected today. The attached space, capacity and enrollment worksheet reflects the enrollment and attendance information along with the staff that were present and caring for children today. At the completion of the visit, I reviewed the Visit Summary with Mrs. Pearsall and computerized copy was given to her. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One teacher was employed prior to receiving their medical assessment. This was corrected because their medical assessment is on file. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. This was not on file prior to employment for one teacher. This was corrected during the visit because it is on file. .0701(a) 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. The enrollment date was not included on the parent's statement. This was corrected during the visit when it was added to the statement. .1804(b) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. This was not on file prior to caring for children for one teacher. The was corrected during the visit when the policy was reviewed with the teacher and signed acknowledgement was on file. .0608(d)(1-4) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Medical information was not maintained separately from staff member's personnel file for all staff. This was corrected during the visit when the medical information for all staff was maintained separate from their personnel file. .0701(d) Because the violations were corrected during the visit you are not required to submit a compliance letter to me. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today’s visit, may negatively affect your compliance score. As of today’s, visit your compliance history is 93%. Mrs. Pearsall also operates a child care center in Cumberland County; Foundations Learning Center ID#26002456 in which she is the administrator. She is required to be on site at that location for at least twenty-five (25) hours a week. I discussed with her that she is required to be on site at this location for at least thirty (30) hours a week. She stated that she is in the process of hiring an administrator for this location. Mrs. Pearsall was required to submit a schedule of how she will meet those hours at both locations to me by April 17, 2024. Rated License Assessment: In order to maintain subsidy payments at the end of the temporary time period, the facility must transition to a three to five star rated license. Education Standards: The education of the Administrator, Lead Teachers, Teachers, and Group Leaders will be assessed for points for the star rated license. It is imperative to submit official transcripts directly to DCDEE and have employees submit all other information using “WORKS” (Workforce online reporting and knowledge system). All staff education should be current before the fifth month of the temporary time period (August 1, 2024) as a rated license will need to be processed before the end date of the temporary license. Program Standards: The center may receive 2 points in program standards by meeting enhanced staff child ratios or enhanced space and by providing the required activity centers and supplemental centers. To receive more than 2 points in program standards, the facility will need to have an Environment Rating Scale (ERS) assessment and score at least a 4.0 on each of the scales. Be sure to visit www.ncrlap.org to familiarize you and your staff with the different assessments. Also, if you meet the requirements for a 4-5 star rated license and plan to provide care to 4 year old’s you will be required to have a curriculum that has been approved by DCDEE. You can review the approved curriculum by visit: https://ncchildcare.ncdhhs.gov/Read/nc-child-care-commission-approved-early-childhood-four-year-old-curricula-2018 Quality Bonus Point: While the facility may meet more than one possible quality point, the facility can only earn a maximum of one quality bonus point. Total points and Star Rated License Information: The total points received in both education and program standards and the quality bonus point will determine the star license issued to your center. To receive a three star license the facility must earn at least seven points. To receive a four star license the facility must earn at least ten points. To receive a five star license the facility must earn at least thirteen points. If you decide to apply for a star rated license you will need to complete the Initial Application for Assessment for a Two Through Five Star Rated License. If you decide to have the ERS assessment conducted, you will need to complete the Rated License Assessment Request Review. These forms were given to you during the visit. They should be completed and submitted to me by April 24, 2024. I also gave you a copy of the Quality Point Form for your review. Technical Assistance: Whether you work with paper documents, electronic files, or a mixture of both, it's vital to keep them organized and accessible. That way you can save time looking for things, and always have the right information to hand when you need it. Ways to organize files: Follow a consistent method for naming your files and folders.
