Your last annual compliance visit was conducted on August 6th, 2024. A checklist was used to note the requirements I monitored today. Today’s visit was conducted with you, Lowanda Kittrell, owner/operator (since your last annual visit you got married and your last name is Frazier. Your FCCH operates with a 4- STAR rated license. Due to current rule making and the QRIS modernization I encourage you to visit the divisions website; https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization , scroll to the bottom of the page and watch the videos and click on the links to read about the changes that are coming regarding rated license. This annual compliance visit was conducted with no children present. You stated you currently have no children enrolled but you will have a child starting next month- August 2025. There are no pets in the home. Aside from yourself your daughter and grandson were present in the home upstairs. All program and provider files were reviewed during today’s visit along with equipment, furnishings, safety requirements, and transportation requirements. You are approved for transportation your license, registration, insurance information was reviewed during today’s visit. Your registration and insurance for the vehicle is current. **VERIFICATION OF RECORDS: *EPR plan was not accessible again and you informed me you had to submit a help request to risk management to receive assistance to access your EPR plan. *Fire drill- 7/25/2025 *Playground inspection 8/25/2025 *Shelter in place/ lockdown drill- 7/25/2025 * The clean water for Carolina Kids requirement for water testing, lead paint and asbestos – not completed and a violation was cited. *ABCMS- not completed and a violation was cited. The following violations were observed during today’s visit. Violation Number Comment Rule 1831 A valid qualification letter was not on file and available for review at the facility. Although a household members qualification was verified through 2031 there was no qualification letter on file for review. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The provider did not complete the required ABCMS criminal record portal. G.S. 110-90.2 & .2703(r) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The operator did not have access to her plan online and required assistance accessing it so updates and revisions could be made. .1714(e ) 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. Your last annual compliance visit was conducted on August 6th, 2024. A checklist was used to note the requirements I monitored today. Today’s visit was conducted with you, Lowanda Kittrell, owner/operator (since your last annual visit you got married and your last name is Frazier. Your FCCH operates with a 4- STAR rated license. Due to current rule making and the QRIS modernization I encourage you to visit the divisions website; https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/QRIS-Modernization , scroll to the bottom of the page and watch the videos and click on the links to read about the changes that are coming regarding rated license. This annual compliance visit was conducted with no children present. You stated you currently have no children enrolled but you will have a child starting next month- August 2025. There are no pets in the home. Aside from yourself your daughter and grandson were present in the home upstairs. All program and provider files were reviewed during today’s visit along with equipment, furnishings, safety requirements, and transportation requirements. You are approved for transportation your license, registration, insurance information was reviewed during today’s visit. Your registration and insurance for the vehicle is current. **VERIFICATION OF RECORDS: *EPR plan was not accessible again and you informed me you had to submit a help request to risk management to receive assistance to access your EPR plan. *Fire drill- 7/25/2025 *Playground inspection 8/25/2025 *Shelter in place/ lockdown drill- 7/25/2025 * The clean water for Carolina Kids requirement for water testing, lead paint and asbestos – not completed and a violation was cited. *ABCMS- not completed and a violation was cited. The following violations were observed during today’s visit. Violation Number Comment Rule 1831 A valid qualification letter was not on file and available for review at the facility. Although a household members qualification was verified through 2031 there was no qualification letter on file for review. G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The provider did not complete the required ABCMS criminal record portal. G.S. 110-90.2 & .2703(r) 1876 The operator did not review the EPR Plan annually or when information in the plan changed to ensure all information is current. The operator did not have access to her plan online and required assistance accessing it so updates and revisions could be made. .1714(e ) 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. The provider did not complete the clean water for Carolina Kids requirement and obtain results for water, lead paint and asbestos. 10A NCAC 09 .1725(a)(2) Technical Assistance: Please remember your ITS_SIDS training has expired and although you currently do not have any children enrolled, you cannot enroll an infant child until you come in compliance by renewing your ITS-SIDS training. Please remember as a licensed provider whether you have children enrolled or not you need to keep in compliance by keeping your program paperwork requirements as well as your provider paperwork requirements up to date at all times. Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a childcare facility. It is also a way to for staff at the CBC Unit to keep you updated of the status of out-of-state checks if a new employee has a Provisional Qualification. Instructions on how to link to the facility were provided today. In the future, new staff need to link within five days of hire or transfer to your center.
