You, Carrie Cranfill, assisted me with the visit. Your program currently operates with a Five Star License effective May 7, 2019. Your last Annual Compliance Visit was conducted on April 18, 2024. Prior to today’s visit your facility had a compliance history of 99%. Upon my arrival, the children were having lunch consisting of cheese burgers, corn, a chilled berry cup, and milk. I observed your Five Star License, Sanitation placard, Emergency Numbers, Emergency Medical Care Plan, tobacco free facility signage, evacuation plans, daily schedules, and activity plans posted as required. Today, children were adequately supervised, enhanced staff-child ratios were maintained, and adequate approved space was used during today’s visit. I observed activity areas including books, blocks, manipulatives, dramatic play, art, and science. I observed toys and equipment to be of sufficient quantity. I observed toys and equipment to be developmentally appropriate in all spaces. I observed electrical wall outlets and power strip outlets covered. I observed the cords secured or inaccessible to children. I observed bathrooms stocked with necessary supplies with handwashing procedure signs posted as required. Inspections/Drills/Logs Sanitation Inspection- September 18, 2024, with a Superior rating and zero demerits. Fire Inspection- November 6, 2024 Playground Inspection- February 25, 2025 Fire Drill- February 21, 2025 Emergency Drill- December 12, 2024 Incident Log- Completed as required. I monitored NC PreK requirements during today’s visit. I observed the Administrator, does not serve as the Lead Teacher or Assistant Teacher in the classroom. I observed a staff-child ratio of one (1) adult to nine (9) children with a group size maximum of two (2) adults to eighteen (18) children maintained. I observed use of the DIAL 4 as the Developmental Screening tool and Creative Curriculum Gold Teaching Strategies as the Formative Assessment tool. You stated you used Creative Curriculum. I observed a white board with 10-15 parent involvement activities listed from throughout the year. I monitored a random selection of two children’s files. I monitored one staff file. And Criminal Background Checks for all staff. Today there was a substitute in the classroom, Sandra Boyette. There was not a qualifying letter on site for her. I checked the ABCMS database and found that there has been no Criminal Record Check completed for Ms. Boyette. I emailed the DCS Preschool Director, Mackenzie Ratledge, to let her know of my findings and that Ms. Boyette would need to leave the facility and will not be able to be utilized in a licensed classroom until she receives a qualifying letter from the Division. Mrs. Ratledge sent another substitute, Wendy Ramsey to the classroom to replace Ms. Boyette. Ms. Ramsey has a valid qualification letter. Your signature on all forms served and will serve as verification the information provided was accurate and complete. There were violations cited during this visit: Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. A substitute staff, Sandra Boyette (hire date 11/22/24) was working in the classroom without having a CBC completed prior to employment. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. A substitute staff, Sandra Boyette (hire date 11/22/24) was working in the classroom without having a qualification letter on file. G.S. 110-90.2(b) & (d) & .2703(e) The following Technical Assistance was provided: 1. I emailed the Preschool Coordinator and suggested she require all Substitutes to carry a file with them containing copies of the following: CBC Qualifying Letter, Verification of CPR/FA Certification, Updated Emergency Information Form, and Recognizing and Responding to Suspicions of Child Abuse and Neglect. I also reiterated the importance of ensuring that substitute staff complete the Criminal Background Check required by the Division prior to being hired as required in rule. 2. An emergency drill will need to take place prior to March 31, 2025. Compliance Plan: All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Provide any documentation that will show compliance with the violation. 7. Please Note: If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. If you cannot meet the requirements by this date, you shall contact me with a timeline of the corrections. In some cases, this timeline may be extended. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time. Consultation was provided on the following: ABCMS As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. QRIS The new QRIS Modernization Plan known as “Pathways to Success” is being added to law which included moving to the use of the updated Environmental Rating Scale (ERS-3 versions) for FCCERS, effective February 1, 2025. DCDEE and the NC Child Care Commission will continue the rulemaking process to add the rule language to child care requirements. This process with take several months. Hold Harmless provisions have been extended. Star rated license reassessments are postponed until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A pathway for child care programs currently accredited through the approved accrediting bodies may now move to a 3- or 5-star license if they are not already at this star level. Providers need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form. Please be reminded any ERS assessment completed on February 1, 2025 and after will be assessed using the ERS-3 versions. You, Carrie Cranfill, Lead Teacher, were not present today. Alisa Allen, Teacher, assisted me with the visit. Your program currently operates with a Five Star License effective May 7, 2019. Your last Annual Compliance Visit was conducted on April 18, 2024. Prior to today’s visit your facility had a compliance history of 100%. Upon my arrival children were laying down to nap. I observed your Five Star License, Sanitation placard, Emergency Numbers, Emergency Medical Care Plan, tobacco free facility signage, evacuation plans, daily schedules, and activity plans posted as required. Today, children were adequately supervised, enhanced staff-child ratios were maintained, and adequate approved space was used during today’s visit. I observed activity areas including books, blocks, manipulatives, dramatic play, art, and science. I observed toys and equipment to be of sufficient quantity. I observed toys and equipment to be developmentally appropriate in all spaces. I observed electrical wall outlets and power strip outlets covered. Sanitation and fire inspections are occurring as required. I observed your last outdoor inspection dated August 29, 2024. I observed that Fire and Emergency drills are occurring as required. There have been no staff changes since my ACV in April. I observed both regular classroom staff to have Recognizing and Responding to Suspicions of Child Maltreatment, and valid CPR/FA. I was able to verify Criminal Record Checks for regular classroom staff. Today there was a substitute in the classroom, Deborah Campbell. There was not a qualifying letter on site for her. I checked the ABCMS data base and found that there has been no Criminal Record Check completed for Ms. Campbell. I emailed the DCS Preschool Director, Mackenzie Ratledge, to let her know of my findings, but was unable to communicate with her prior to the end of my visit. Ms. Campbell will not be able to be utilized in a licensed classroom until she receives a qualifying letter from the Division. I was unable to locate her name on the Public-School Off-Site Verification Records for Staff. I contacted the DCS Preschool Director who was able to send me, via email the items from Ms. Johnson's file that I needed for this visit. There was one violation cited during this visit: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. A substitute staff member did not have a file available for review by the consultant. G.S. 110-91( 9) The following Technical Assistance was provided: 1.
