I used the "Annual Compliance Monitoring Checklist for Child Care Centers" during today's visit. Your last annual compliance visit was conducted on April 11, 2024. Your facility’s compliance history score prior to today’s visit was 99%. You, Nikia Green, Lead Teacher, assisted me with today’s visit. I observed the Five Star License, NC Child Care Summary of the Law, Safe Procedures for Arrival & Departure, current menus, Sanitation placard, Emergency Numbers, Emergency Medical Care Plan, daily schedules, activity plans, and staff-child ratio worksheets, First Aid poster, tobacco free policy signage and evacuation plans posted as required. I observed children adequately supervised, adequate approved space use and permit restrictions maintained during today’s visit. Today children were engaged in free play in activity areas, teacher directed math activities, outdoor play, and story time. The classroom had age-appropriate child-sized tables, chairs, shelving, and storage provided. I observed developmentally appropriate materials provided in sufficient quantities. I observed all materials, furniture, and equipment to be in good repair. Bathrooms were stocked with necessary supplies and handwashing procedure signs posted at the classroom and handwashing sink areas. The outdoor play area was monitored. You stated none of the children had medications. You stated your program does not provide transportation. Drills/Inspections/Logs Fire Drill-February 28, 2025 Emergency Drill-February 21, 2025 Sanitation Inspection-September 25, 2024, with a Superior rating and four demerits. Fire Inspection- November 12, 2024 Playground Inspection- March 4, 2025 Incident Log-Completed as required. I monitored NC PreK requirements during today’s visit. I observed the Administrator, does not serve as the Lead Teacher or Assistant Teacher in the classroom. I observed a staff-child ratio of one (1) adult to nine (9) children with a group size maximum of two (2) adults to eighteen (18) children maintained. You stated you used Dial-4 as the Developmental Screening tool and Creative Curriculum Gold Teaching Strategies as the Formative Assessment tool. You stated you used Creative Curriculum. This school year some of the parental activities have been parent/teacher conferences, family poster take home projects, parents have been invited to school activities, newsletters and a graduation ceremony will be held. I monitored a random selection of two children’s files. I observed the Staff Records Offsite Verification Form for this school year on file in the classroom. There have been no new staff at this facility since my last visit. I monitored one staff file. Your signature on all forms served and will serve as verification the information provided was accurate and complete. The following violations were observed or cited and corrected during this visit: Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. A screen time log was not available for review during the visit and screen time was not documented on the activity plan. .0510(d)(2)(A-C) Compliance Plan: No compliance plan is due at this time because you were able to correct the violation during the visit. Technical Assistance was provided on the following: 1. Screen time may be documented on your activity plan or on a separate log. You were able to print off a log from the division website during the visit to use. Consultation was provided on the following: ABCMS As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. QRIS The new QRIS Modernization Plan known as “Pathways to Success” is being added to law which included moving to the use of the updated Environmental Rating Scale (ERS-3 versions) for FCCERS, effective February 1, 2025. DCDEE and the NC Child Care Commission will continue the rulemaking process to add the rule language to child care requirements. This process with take several months. Hold Harmless provisions have been extended. Star rated license reassessments are postponed until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. A pathway for child care programs currently accredited through the approved accrediting bodies may now move to a 3- or 5-star license if they are not already at this star level. Providers need to submit a request to their licensing consultant and provide a copy of their accreditation award/certificate for processing: Accreditation Licensure Request Form. Please be reminded any ERS assessment completed on February 1, 2025 and after will be assessed using the ERS-3 versions. You, Nikia Green, Lead Teacher assisted me with the visit. Your last Annual Compliance Visit was conducted on April 11, 2024. Prior to today’s visit your facility had a compliance history of 100%. I observed your Five Star License, Sanitation placard, Emergency Numbers, Emergency Medical Care Plan, tobacco free facility signage, evacuation plans, and daily schedules, posted as required. Today children were adequately supervised, enhanced staff-child ratios were maintained, and adequate approved space was used during today’s visit. I observed children engaged in free play in activity areas, transitions, and whole group activities. I observed activity areas including books, blocks, manipulatives, dramatic play, art, and science. I observed toys and equipment to be of sufficient quantity. I observed toys and equipment to be developmentally appropriate in all spaces. General safety was monitored throughout the classroom. There are currently no children enrolled with medications. I observed bathrooms stocked with necessary supplies with handwashing procedure signs posted as required. Inspections/Drills/Logs Sanitation Inspection- September 25, 2024, with a Superior rating and 4 demerits. Fire Inspection- September 26, 2023 Playground Inspection- October 2, 2024 Fire Drill- October 14, 2024 Emergency Drill- September 4, 2024 Incident Log- Maintained as required. There have been no staff changes since my ACV in April. I observed all staff to have Recognizing and Responding to Suspicions of Child Maltreatment, and valid CPR/FA. I was able to verify Criminal Record Checks for all staff. Your signature on all forms served and will serve as verification the information provided was accurate and complete. There was one violation cited during the visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection conducted was September 2023. 10A NCAC 09 .0304(a) Technical Assistance was given for the following: 1. I suggested putting a reminder in the calendar to schedule the fire inspection at least 1 month before the day it expires. Since you have a detached classroom, you may have your inspection independent of the school. Compliance Plan: All violations cited must be corrected immediately. Please submit a written, signed, and dated statement to me at the email/address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. Address your plan to ensure that you will not have that violation again. 6. Your signature I advise you to be prepared to provide supporting documentation in the form of photos, documents, certificates, etc. should I request them in order to clarify or verify compliance.
