On 10/14/2021, I completed an unannounced onsite investigation at the center. I interviewed Ms. Voss and Ms. Marshall. Ms. Marshall stated she and Ms. Voss was made aware by Ms. Cook and other child care staff members that Child A was taken out of the preschool classroom and brought to the office when he was having behavior problems. Child A would remain in the office until he calmed down. Unknown the amount of time he would spend in the office. I informed Ms. Marshall and Ms. Voss that the office was not approved child care space and that unless the space/room is approved could not be used. Ms. Marshall did not know why Child A was not given the option for quiet activities during nap times to avoid loud outbursts. I discussed with Ms. Marshall and Ms. Voss that children were not required to sleep during nap time and quiet activities or options should be provided to children who don't sleep. Child A returned to the center on 10/11/2021. His parents brought a "quiet box" which have different activities he can use while lying on his cot during nap time. On 10/14/2021, I interviewed Ms. Cook by telephone. Ms. Cook stated Child A often did not like to sleep during naptime. She often went into the preschool classroom to assist as some of the children in the room do not settle down quickly. Child A wanted to get up from the cot. When Child A is up, he will run around the room, jump on cots, and kick cots to wake others in the room. I questioned why on 10/06/2021, Child A was not given quiet activities, to do during nap time as he is not required to sleep. She was attempting to keep Child A's behavior from escalating as they become more pronounced when he does not sleep. On 12/06/2021, I completed the Exit Conference by telephone with Ms. Jacquelyn Voss, owner. I informed her of the violations being cited in the investigation and that she would receive a copy of the report, once approved. On 12/28/2021, email sent to Ms. Marshall requesting contact. On 12/29/2021, I spoke with Ms. Marshall by telephone. I informed Ms. Marshall that Ms. Voss was not in the system as the licensee designee. She would need to be identified as the licensee designee and connected in the Child Care Background Check system under this center's license. Ms. Marshall knew that Ms. Voss was connected to one of the other centers they owned. I relayed that I would need to complete the exit conference with Ms. Comparoni instead as the identified licensee designee. On 12/29/2021, I received a telephone call from Ms. Comparoni, licensee designee. I informed Ms. Comparoni of the violations being cited in the investigation.
