On 05/24/2023, I spoke with Ms. Jeanine Beck, licensee designee/program director by telephone. Ms. Beck stated there was a recent incident on 05/23/2023, where Child A was left alone in her classroom. She was informed by Child A's Mother of the incident and not by the child care staff members. Ms. Beck is currently gathering all of the information, statements and speaking with everyone involved. I informed Ms. Beck I would need to interview all child care staff involved. On 05/25/2023, I completed an onsite investigation. I interviewed Ms. Jeanine Beck, licensee designee/program director, Child Care Staff member 1, Child Care staff member 2, and Witness 1. I interviewed Witness 1. Witness 1 stated at approximately 11:15 a.m. they were delivering lunch to the classroom, #105. The door was locked, she unlocked the door, walked in with the food. Placed the food down and saw Child A standing in the corner crying. Witness 1 did not initially see or hear Child A went entering the room. Witness 1 told Child A she would get her to her classroom. Child A was still crying as she leads her outside to Child care staff member 1 and Child care staff member 2. Child care staff member 1 said, "Oh no" when she saw her bringing Child A outside. Child care staff member 1 started to console Child A. Witness 1 left Child A and returned to completing her task. Witness 1 been working for the center for the last two-three years. I interviewed Child care staff member 2. Child care staff member 2 stated this is the first year she has worked in the classroom #105. Child care staff member 2 called the children at approximately 10:45 a.m. to line-up as there were approximately 17 children in the classroom and 10 of the children were coming with her. Child care staff member 1 was coming with the remainder of the children. She went outside as soon as Child care staff member 1 arrived, passed her the chart with the children's name to check the children off the list. They have the children to line up against the wall and as their name is called and the children are allowed to play. She was standing in the play area as Child care staff member 1 called the children's name. She was not aware that Child A was left behind in the classroom until Witness 1 came outside talking with Child care staff member 1 and Child A was with her. She asked Child care staff member 1 why Witness 1 was outside, and she was told by Child care staff member 1 she "forgot Child A inside the classroom. Child care staff member was shocked that this happened and shortly after gathered the children to back inside. She thought with Child care staff member 1 being the last person out the classroom, she would make sure all of the children were with her. Child care staff member 3 Child care staff member 2 did not alert Ms. Beck of this matter as she thought this incident would have been reported by Child care staff member 1 as she reported she would tell Child A's Mother and Ms. Beck. I interviewed Child care staff member 1. Child care staff member 1 stated she been working for the center for the last 5 years. She could not recall the time this incident occurred it during the time for their scheduled outside play time. She was writing a note for the custodian at her desk. There were 17 children in the classroom, and it was a little chaotic with the kids playing as Child care staff member 2 was getting them lined up to transition outside. All of the children were walking out with Child care staff member 2. She typically turns around in the classroom to ensure all of the children are out of the classroom and did not see that Child A was in the classroom. Left out of the classroom with two children vying to hold her hand as they walked down the hallway. There is one staff at the front of the line and the second staff in the rear.
