    R 400.8125 · 1 R 400.8125 Staff; volunteer; requirements. (1) All staff and volunteers shall provide appropriate care and supervision of children at all times.

    On 05/15/2023, I spoke with Ms. Jeanine Beck, licensee designee by telephone. Ms. Beck stated there been an incident at Amanda Moore's SACC program. Child A was missing from the program and child care staff members were unaware Child A was missing until Child A's father arrived for pick-up. Child Care staff member spoke with Witness 1 and was told Child A was sent to his "normal school bus" for transport after trying to contact Child A's Mother. Witness 1 did not contact Child A's Mother but sent Child A to the bus as that was his normal routine. Child A was located at his mother's previous home with a neighbor. Child A was missing for approximately 45 minutes. Ms. Beck would forward statements and incident report about this matter. On 05/15/2023, I received an email from Ms. Beck with Witness 1 statement. The statement stated the following: Child A arrived in the office on 05/12/2023 asking if he was a parent pick-up. Witness 1 attempted to contact Child A's Mother by telephone and left a voice message stating Witness 1 was sending Child A on the bus and to contact if there were any questions. Child A was sent outside to bus 24. Around 4:30 p.m. several people arrived in the office including Child A's Father. Child A's Father acknowledged Child A's Mother no longer lived at that address and he was contacting 911. Witness 1 contacted the school transportation which confirmed with the bus driver that Child A was dropped off at the Clubhouse. On 05/15/2023, I received an email with from Ms. Beck with Witness 2 statement. Witness 2 statement stated as they were walking towards the office, she observed Child A's Father talking with Child Care Staff member 2, discussing Child A's whereabouts. Child Care Staff member 2 explained Child A checked into the SACC program, was told to put his stuff down and sign in. Unbeknownst to Child Care Staff 2, Child A went to the office where he was sent to the bus. Witness 2 walked into the office and found Witness 1 on the phone with transportation. Transportation confirmed Child A was dropped off at his listed living location. Child A's Father was upset, and that Child A was to go to SACC and should have known not to take the bus as he had not taken the bus all week. Child A's Father was also upset that staff did not know he was missing. Police directed Child A's Father to go Child A's bus drop off where Child A was found safe with a neighbor by is house. I interviewed Child A's Father on 05/15/2023, by telephone. Child A's Father stated Child A recently signed-up for the SACC program as his first day of attendance was May 8, 2023. Child A would be attending every other week. At approximately 4:30 p.m. he arrived at the center for pick-up, went in to sign him out and did not see him. 3 He asked where Child A was and at first it was thought he was in the bathroom or the cafeteria. Child A's Father went to the office and Witness 1 told them that Child A went home on the bus. Child A's Father stated he was very upset of the following: none of the staff were aware Child A was not in the program (missing), no one contacted him from the office and that Child A should have known not to take the bus as they discussed pick-up on 05/12/2023 in the morning prior to him dropping Child A off to the SACC program. The police directed him to go to the drop-off site. Child A's Father found Child A with a neighbor. Child A's Mother recently moved out of the district and that information was not known by the school. Child A's Father would ensure that his phone number was available for contact in the front office, and he would talk with Child A at drop off in the morning to ensure that he knows the pick-up plan. Child A would be returning to the SACC program. On 05/18/2023, I completed an unannounced onsite investigation and interviewed Child Care Staff member 1 and Child Care Staff member 2.
