There was no Health Inventory Part II nor was there a lead screening in one of the children's files. The provider stated that the parent stated she would be dropping the form off the following day because the form was originally missing information and the provider gave the form back to the parent. It is recommended to make a copy of a child's record/document when a parent is updating the form for future reference.
The provider needs to submit 7 hours of continued training for the 10/23 - 10/2024 timeframe. Also as a reminder, also submit 12 hours of continued training for the 10/24 - 10/2025 timeframe.

The provider agreed to perform the fire/emergency drills as required. The sheet that was displayed in the child care area for the 2023 year, did not contain the date and time of drills. The provider stated fire drills were conducted, but she would need to look for the sheet as documentation. The provider agreed to submit a copy of the fire drill log to the Licensing Specialist.
