There is one child in care whose emergency form is missing the doctor's contact information. There is one child in care whose emergency form is missing a daily authorized pick up person listed on the emergency form. Please have the parents complete these sections and send a copy to the OCC.

The provider has an above ground pool surrounded by a deck. The gate to the pool is closed and locked - when the Licensing Specialist unlocked and opened the gate, the gate would not self close or self latch properly. The two side gates to the deck steps are also not self closing or self latching properly. The alarm on the inside door is not working. Please ensure that all gates are self closing and self latching properly. Please ensure that the alarm is working properly. Please inform the OCC when these have been repaired.
There is one child who is missing updated immunizations. Please ensure that the parents obtain a copy of the updated immunizations for the child's file.
There is one child who is 2 years old who does not have evidence of lead blood testing in her file. Please have the parents obtain this information.
The provider does not have parents sign the children in and out, as the provider does it, and does not have the parents verify the attendance record. The provider is advised to have parents sign or initial verifying the children's attendance.
There is one child enrolled whose emergency form does not have at least one emergency contact person listed. Please ensure the parent lists at least one emergency contact person and their contact information in the event that a parent can not be reached.
The provider has several open litter boxes in the living room on the main floor of the home. The provider indicates that the children spend most of their day downstairs in the basement playroom. The provider uses the dining room on the main floor for eating and play. The provider uses the living room on the main floor for napping. She indicates that she moves the litter boxes before the children lay down for napping. The provider also has a stray kitten in the bathroom on the main floor and has a litter box in there. The provider is advised that she may not have litter boxes in the child care areas. The provider indicated that once the kitten is seen by the vet and able to be vaccinated, it will be in the house with the other cats. The provider will move the litter boxes so they are inaccessible to the children. There is mold on the ceiling around the vent in the basement bathroom. The provider is advised to clean the mold, clean the vent, and ensure that the vent is working properly to prevent the mold from returning.
There are several outlets in the basement that are not capped. Please ensure that all outlets that are accessible to the children in the child care areas are capped at all times.
The provider does not have a door knob cover on the door at the top of the basements steps. The provider is advised to place a door knob cover on that door knob to prevent children from accessing the basement stairs.
The provider has an above ground pool that is surrounded by a deck with railing. The pool deck is separated from the rest of the back deck. There is an access gate to the pool from the back deck that is locked. The Licensing Specialist unlocked the padlock with a key and tested the gate. The gate is not self closing and self latching properly. The provider is advised to adjust, repair, or replace the gate so that it is self closing and self latching properly.
The provider did not complete the 2024 annual update. The provider is advised to complete this training and submit the certificate as soon as possible.
